This is to inform you that the last day to request your child's medical records is Friday, July 25th. You must come in the office to fill out release form. You will then be notified when they are ready for pickup. It is taking about 2 weeks to process.We will not accept any new request after this date. Thank you.

Clinic hours are Monday thru Thursday 8am to 11:30am and 1:00pm to 4:00pm. Closed on Friday.

UPDATED INFORMATION...PARENTS WILL STILL NEED TO COME INTO THE CLINIC TO SIGN FOR THIER CHILD'S MEDICAL RECORDS. WE WILL CALL YOU ONCE THE DISC IS READY FOR PICK UP..EVEN THO DR. WHATLEYS LAST DAY IS TOMORROW, WE WILL STILL BE OPEN TILL END OF JULY FOR RECORD PICK UP. PLEASE CALL 918-687-4411 IF YOU HAVE ANY QUESTIONS. WE ARE TRYING TO MAKE THE TRANSITION AS EASY AS POSSIBLE.

IMPORTANT POST. Dr. Whatley has accepted a position at NEO Health here in Muskogee. His last day at the Clinic will be June 26th. He will begin seeing patients at NEO around July 15th. If you choose to follow him, we ask that you come to the Children's Clinic as soon as possible to request your child's records. All request must be made in person not over the phone . All records will be put on a disc. The Children's clinic will close once he leaves. It has been an honor to serve Muskogee and it's little ones. Please call 918-687-4411 with any questions.

This a reminder with summer sports approaching. If your child needs a sports physical, they MUST be up to date on their annual well child. These visits CAN NOT be combined in to one visit. You will have the well child and then will be scheduled for the sports physical. This is why it is so important to keep the well child's up to date. Also, if you need a shot record, please keep in mind that there is a 24-48 hour turn around for those to be processed. These must be picked up in person or we can mail them.
