When you have a life-limiting illness, Embrace Hospice offers compassionate, expert care to help you to live each day to the fullest. We strive to enhance the quality of the lives of the patients and families we serve, and emphasize patient choice and dignity. Embrace Hospice uses a team approach to care for the physical, spiritual, emotional, and social needs of the patient and their family. The patient and family always remain in control of the plan of care, with their nurse serving as their team leader. Team members include medical directors, registered nurses, certified nursing assistants, social workers, chaplains, bereavement coordinators, volunteers, and many other discipline staff members to design and implement care plans specific to you or your loved ones. Embrace Hospice serves patients living in 23 South Carolina counties and 15 Georgia counties.
