We are excited to launch our page dedicated to supporting the health and well-being of our community in Napa County! Our mission is to provide Enhanced Care Management (ECM) and Community Health Worker (CHW) services tailored to your needs, ensuring that everyone has access to the care they deserve.

✨ What We Offer:

Enhanced Care Management (ECM) Services Include:
- Transportation Services: Reliable transportation to your medical appointments, ensuring you never miss essential care.
- Connecting with Doctors: Assistance in finding and connecting with healthcare providers for a smoother healthcare journey.
- Applying for Community Resources: Guidance in navigating and applying for resources available in our community, including financial assistance and health programs.
- Personalized Care Coordination: Our dedicated care coordinators develop personalized care plans that meet your unique health needs.
- Education and Support: Resources and support to empower you in managing your health.

Community Health Worker (CHW) Services:
- Preventive Health Outreach: Helping patients address missing care gaps.
- No-Show Follow-Up: Assisting patients who miss appointments to ensure they receive necessary care.
- Social Care Coordination: Connecting patients with housing and food resources.
- Chronic Care Management: Supporting individuals with chronic conditions like obesity, diabetes, and hypertension.
- Individual Health Education: Providing health education for better self-management.
- Prenatal and Postnatal Care Coordination: Ensuring mothers receive the necessary care before and after childbirth.
- Transitional Care Coordination: Assisting patients post-emergency department visits, hospitalizations, or detoxification.

Who We Serve:
We are proud to serve patients enrolled in Medi-Cal Partnership Health Plan of California. Our population of focus includes:
- Individuals experiencing homelessness
- High hospital or emergency department utilizers
- Individuals with serious mental health and/or substance use disorders
- Justice-involved individuals transitioning from incarceration
- Vulnerable adults living in the community at risk for long-term care
- Adult nursing facility residents transitioning to the community
- Children and youth enrolled in CCS services
- Children and youth involved in child welfare
- Individuals with intellectual or developmental disabilities
- Birth equity population
