Did you know? 62% of patients found significant relief at 2 weeks, with lumbar blocks lasting 6+ months.

When back or neck pain persists despite other treatments, identifying the exact source becomes critical. Medial branch blocks serve a dual purpose: pinpointing whether facet joints are causing your pain and providing temporary relief.

The Evidence Behind Medial Branch Blocks

Cervical medial branch blocks achieved 62.2% clinically significant pain reduction at 2 weeks, declining to 22.7% at 3 months. This demonstrates valuable diagnostic and short-term therapeutic benefit.

Lumbar medial branch blocks showed even more promising results. Therapeutic benefits lasted at least 6 months post-injection with statistically significant improvements in both physical and emotional functioning.

Who Benefits from This Procedure?

• Patients with suspected facet joint-mediated pain in the cervical, thoracic, or lumbar spine
• Those requiring diagnostic confirmation before considering radiofrequency ablation
• Individuals seeking temporary relief from facet joint arthritis or spinal osteoarthritis
• Candidates who achieve greater than 50% pain relief qualify for longer-lasting radiofrequency treatment

What to Expect

Medial branch blocks are performed as outpatient procedures using fluoroscopic guidance to ensure precise placement of local anesthetic at the medial branch nerve locations. Multiple spinal levels may be blocked during a single session. The procedure helps determine if facet joints are your pain source while providing temporary relief.

At Napa Valley Orthopaedic Medical Group, Dr. Jackie Weisbein combines advanced interventional techniques with personalized care. Every treatment plan starts with understanding your unique pain story.

Did you know most herniated discs heal without surgery when treated conservatively?

It's true. Conservative management can effectively alleviate symptoms and promote spontaneous resorption of herniated disc material.

At Napa Valley Orthopaedic Medical Group, we start with evidence-based conservative care for herniated discs. Clinical guidelines confirm that conservative treatment is the first-line approach for lumbar disc herniation and includes pharmacological therapy, physical therapy such as mechanical traction, and rehabilitation.

The majority of herniated discs resolve with this approach. Your body has remarkable healing capacity when given the right support.

Who Benefits from Conservative Treatment?

• Patients with herniated disc diagnosed on MRI
• Those experiencing sciatica or nerve compression symptoms
• Individuals with radicular pain, sensory changes, or motor weakness
• Anyone seeking to avoid surgery when possible

Conservative care may include medication management, targeted physical therapy, and rehabilitation tailored to your specific condition. For patients requiring additional intervention, epidural steroid injections can be performed as an outpatient procedure under fluoroscopic guidance to reduce inflammation around neural structures.

Dr. Weisbein takes a personalized approach to each patient's care. We understand the frustration of persistent pain and create treatment plans aligned with your goals and lifestyle.

Many patients find relief without needing surgery. Conservative treatment gives your body the best chance to heal naturally while managing symptoms effectively.

Over 80% of patients who tried it said it helped a lot or they loved their pump.

When chronic pain persists despite every other treatment, hope can feel distant. An intrathecal pain pump offers a different path.

This implanted drug delivery system places medication directly where it's needed, at spinal cord receptors. Because the medication bypasses systemic circulation, patients require significantly lower doses than oral medications.

The evidence is compelling. In a study of 533 patients, 81% reported satisfaction: 41.7% loved their pump and 39.2% said it helped a lot. Only 1.3% (just 7 patients) reported the device wasn't helpful or regretted the implant.

This technology has been used since the early 1980s, with consistent efficacy data spanning over 40 years and a well-established safety profile.

Who May Benefit?

• Severe chronic pain that hasn't responded to conventional treatments
• Cancer pain requiring high-dose opioid therapy
• Failed back surgery syndrome not responding to other options
• Spasticity from neurological conditions
• Patients willing to attend periodic refill appointments

The system includes a programmable pump placed under the skin (typically in the abdomen) and a catheter positioned in the intrathecal space. The pump is refilled periodically through an access port during office visits.

At Napa Valley Orthopaedic Medical Group, Dr. Jackie Weisbein provides boutique, personalized care. She understands the frustration of feeling like you've tried everything. Her approach combines advanced interventional techniques with holistic support.

What if one outpatient procedure could quiet your facet pain for 6 to 12 months or longer?

For patients with chronic facet joint arthritis or medial branch nerve-mediated spinal pain, radiofrequency ablation offers a proven path forward when conservative treatments haven't been enough.

How RFA Works:

Radiofrequency ablation uses radiofrequency energy to heat and disable pain-transmitting nerves. For facet joints, medial branch nerves are targeted with precision.

The procedure is performed as an outpatient visit using local anesthesia with conscious sedation. Fluoroscopy (real-time X-ray guidance) ensures accurate needle placement at the nerve location.

A thin needle is placed near the target nerve. Radiofrequency current heats the nerve tip to 80-90°C for 60-90 seconds, disrupting pain signals without affecting movement or sensation.

Who May Benefit:

• Patients with facet joint-mediated pain or SI joint pain
• Those who achieved greater than 50% pain relief from diagnostic medial branch blocks
• Chronic pain from osteoarthritis in the spine
• Pain unresponsive to physical therapy and medications

Pain relief typically lasts 6 to 12 months or longer. The procedure can be repeated if pain returns.

At Napa Valley Orthopaedic Medical Group, Dr. Jackie Weisbein takes a personalized approach to interventional pain management. Board-certified in Physical Medicine and Rehabilitation and Pain Medicine, she understands the frustration of trying everything without lasting results.

You deserve care that listens to your story and aligns treatment to your goals.

When conservative treatments stop working, you need options that actually help.

If you've tried physical therapy for SI joint pain without lasting relief, you're not alone. Many patients reach a point where they need more targeted intervention.

Research shows real promise. A 2023 randomized controlled trial by Patel found that intra-articular corticosteroid injections reduced pain scores from 6.97 to 3.06 at one month (p

Think TENS is just a placebo? 381 studies with 24,543 patients say otherwise.

If you've been told TENS therapy doesn't really work, you're not alone. This common misconception keeps many patients from exploring a safe, non-invasive option for chronic pain management.

Let's set the record straight.

❌ MYTH: TENS is just a placebo with no real pain relief.

✅ FACT: A systematic review of 381 randomized controlled trials involving 24,543 patients showed statistically significant pain reduction compared to placebo (SMD -0.87). Additionally, the Johnson 2022 systematic review demonstrated that TENS reduced chronic pain intensity versus placebo in 1,417 patients across 31 RCTs.

This isn't anecdotal evidence. This is rigorous scientific data showing real pain reduction in real patients.

TENS therapy uses transcutaneous electrical nerve stimulation delivered through surface electrodes placed on the skin. It modulates pain signals without surgery, injections, or implantation. The therapy is typically well-tolerated and can be used as part of a comprehensive pain management approach.

Who May Benefit from TENS Therapy?

• Patients seeking non-invasive pain management options
• Those with chronic pain conditions looking for alternatives to medication
• Individuals who prefer conservative treatment approaches
• Patients exploring adjunct therapies alongside other interventions

At Napa Valley Orthopaedic Medical Group, Dr. Jackie Weisbein takes a holistic, personalized approach to pain management. TENS therapy may be one component of a comprehensive care plan tailored to your unique needs and goals.

You deserve accurate information about your treatment options.

Did you know? 90 studies over 16 years confirm epidural injections work for radiculopathy and spinal stenosis.

If you've been living with sciatica, leg pain, or numbness from a herniated disc or spinal stenosis, you're not alone. And you're not out of options.

The American Academy of Neurology published an updated systematic review in February 2025 after analyzing evidence since their 2007 assessment. The findings? 90 studies over 16 years showed both short-term (≤3 months) and long-term (≥6 months) improvements for patients with radiculopathy and spinal stenosis.

This is real evidence supporting a treatment that can reduce inflammation around nerve roots and provide meaningful relief.

Who May Benefit from Epidural Steroid Injections?

• Patients with radicular pain (sciatica) from herniated disc or spinal stenosis
• Those experiencing leg pain, numbness, or tingling from nerve root inflammation
• Patients seeking to reduce inflammation around neural structures
• Appropriate for cervical, thoracic, or lumbar radiculopathy
• May be used as part of a comprehensive treatment plan including physical therapy

What to Expect

Epidural steroid injections are performed as an outpatient procedure using fluoroscopic (X-ray) guidance to ensure accurate medication placement in the epidural space. The injection combines a corticosteroid to reduce inflammation around nerve roots with a local anesthetic for immediate pain relief. A series of injections may be administered depending on response to treatment.

At Napa Valley Orthopaedic Medical Group, Dr. Jackie Weisbein takes a personalized approach to pain management. She understands the frustration of trying everything without results. Her goal is to help you reclaim the activities you love through evidence-based interventional care.

The 2025 review of 15 studies found kyphoplasty superior to conservative treatment for up to 12 months.

For patients living with vertebral compression fractures from osteoporosis or trauma, the pain can be debilitating. Severe back pain with weight-bearing activities, progressive height loss, and reduced mobility can steal your quality of life.

Kyphoplasty offers a proven path forward.

Balloon kyphoplasty showed superior pain reduction compared to conservative treatment from 1 to 12 months and improved disability at 1 and 3 months, with moderate certainty evidence (2025 Systematic Review and Meta-analysis). This isn't anecdotal. This is rigorous research showing real relief.

Who May Benefit from Kyphoplasty?

• Patients with painful vertebral compression fractures from osteoporosis
• Those with vertebral fractures from trauma
• Individuals experiencing acute onset severe back pain with weight-bearing activities
• Patients with progressive height loss or spinal deformity
• Those whose pain has not adequately responded to conservative treatment

What to Expect:

Kyphoplasty is a minimally invasive procedure performed under fluoroscopic guidance with conscious sedation or general anesthesia. A balloon tamp is inserted into the fractured vertebra and inflated to restore vertebral body height, then bone cement is injected to stabilize the fracture. The procedure is typically performed on an outpatient basis with same-day discharge.

At Napa Valley Orthopaedic Medical Group, Dr. Jackie Weisbein brings Board Certification in Physical Medicine & Rehabilitation and Pain Medicine, with Fellowship training in Interventional Pain Management. Her approach is personal, evidence-based, and focused on helping you reclaim what pain has taken.

What if pain relief could last 20+ years, not just months?

For patients with chronic pain who feel they've exhausted their options, spinal cord stimulation offers evidence-based hope. This advanced therapy delivers electrical impulses to the epidural space near the spinal cord, modifying pain signals before they reach the brain.

The research speaks for itself. A 20-year outcome study of 224 consecutive failed back surgery syndrome patients showed long-term effectiveness (Nissen 2019, Neurosurgery). In another study, 50 of 65 trial patients received permanent implants for post-operative chronic radicular pain (Hara 2022).

Who Benefits from Spinal Cord Stimulation?

• Patients with failed back surgery syndrome
• Those with Complex Regional Pain Syndrome (CRPS)
• People with chronic neuropathic pain unresponsive to conservative treatments
• Individuals seeking to reduce pain medication use

The process begins with a trial period to ensure effectiveness before permanent implantation. The implanted pulse generator is typically placed in the lower back or buttock area, and you control the stimulation via remote control. Most patients go home the same day, with recovery taking approximately 2 weeks before resuming normal activities.

At Napa Valley Orthopaedic Medical Group, Dr. Jackie Weisbein takes a boutique, highly personalized approach to interventional pain management. She understands the frustration of feeling like nothing works and missing out on life's moments because of pain.

Intracept procedure uses a radiofrequency probe to target vertebrogenic low back pain with minimally invasive precision.

Chronic pain can feel endless. Many patients with chronic low back pain spend years searching for answers, feeling frustrated and unheard when medications, injections, and physical therapy come up short. Inspired by their perseverance, we are proud to offer a next-level solution at Napa Valley Orthopaedic Medical Group.

The Intracept procedure, also called basivertebral nerve ablation, is a minimally invasive treatment for specific cases of chronic low back pain that haven’t responded to traditional therapies. This approach is designed for those who experience vertebrogenic pain (often confirmed by imaging) and feel they’ve tried everything.

What makes the Intracept procedure unique? Here’s our simplified breakdown:
- A small incision is made in the back
- A thin, hollow cannula is guided into the vertebra
- A channel is created to the basivertebral nerve (the nerve that carries pain from irritated vertebral endplates)
- A radiofrequency probe is inserted to ablate the basivertebral nerve, reducing pain signals at the source

It’s usually completed in about 60 to 90 minutes, under general anesthesia, so patients head home the same day once sedation wears off. Post-procedure, mild soreness or swelling near the incision can happen, and we recommend resting for around 48 hours. Most people are back to daily activities soon after, avoiding vigorous exercise for two to four weeks.

Clinical studies show improvements in patient-oriented outcomes after basivertebral nerve ablation, yet we stay committed to evidence, connection, and personalized guidance for every journey.

We know how important it is to feel seen and supported. Our team, led by Dr. Weisbein, specializes in holistic, tailored care to help patients get back the things they love. Sometimes, hope really does return. 💫

What causes sciatica and what are the most effective sciatica treatment options for lasting relief?

Sciatica is more than just back pain. If you’ve ever felt shooting pain down your lower back, tingling in your legs, or loss of balance, you know how disruptive it can be. For many, it feels like life is on pause.

Your sciatic nerve is the largest nerve in your body. When it’s compressed or irritated, symptoms can flare. Common causes include a herniated disc in the lower spine (the most frequent reason), spinal stenosis, spondylolisthesis, degenerative disc disease, muscle spasms or a tight piriformis muscle, and sometimes back injuries or overexertion at the gym.

The goal? Treatment has two goals: to alleviate your symptoms and relieve the compression on your sciatic nerve. Every plan is personal and designed for long-term relief, not just a quick fix.

Here’s how care can look:
- Conservative options like anti-inflammatory medication
- Steroid injections, reducing swelling and pain
- Physical therapy focused on movement and stability
- Advanced options (when needed), such as nerve blocks or spinal cord stimulation, especially if pain persists or there’s permanent nerve injury

There’s evidence to support nonsurgical interventions for new or long-standing sciatica. Both physiotherapy and combined conservative methods show benefit, with approaches that adapt to your unique story.

Most importantly, expect a customized, multimodal treatment plan focused on your goals and challenges. You’re not alone in this. At Napa Valley Orthopaedic Medical Group, care means taking every frustration seriously and helping patients reclaim parts of their lives once limited by pain.
