Recently, one of our clinicians connected with a member in California who had just been discharged and was confused about her medications.
The member’s sodium levels had already been low, and the discharge paperwork didn’t match what she remembered the attending physician saying.
Additionally, her P*P was out of town for weeks, and as a primarily Spanish-speaking member, she didn’t know who to turn to for clarity.

• Spoke directly with the attending physician
• Confirmed which medication needed to be stopped
• Explained everything in Spanish
• Helped her feel confident and safe during a vulnerable transition
