πŸ”Έ This is a continuation post from the case of yesterday ;
β–ͺ️ The answer is no, we cannot apply the esthetic parameters when looking inside the mouth only. We must put a face to the teeth to be able to measure anatomical landmarks and set the starting point;
β–ͺ️ By looking at the photo above , we would be lost to set the correct position of the teeth in harmony with esthetics and function ;
β–ͺ️ This is a typical case of inverted smile when the pre-maxilla has lost bone volume and the maxillary posterior segment have supra-erupted due to lack of mandibular occlusal support. So how do we fix this mess ?
β–ͺ️ By putting a face and smile to the photo above we can start gauging where the future maxillary incisors would be as you starting point for the smile design and occlusal plane . From there we just remove what is in our way and add what is missing;
πŸ”Ή This is a combination of conventional maxillary denture after extraction and aggressive bone plasty (see photos), opposing a mandibular locator (no hole no screws) fixed complete (see video and photos);

🀨 The common thought is changes in occlusal vertical dimension (OVD) and TMJ issues;
πŸ€“ Occlusal analysis, Mahan test results , Range of motions are all necessary evaluation of the static and dynamic occlusion;
🧐 Functional analysis are important to evaluate restorative space and start making decision of changes in occlusal vertical dimension;
πŸ€” Other test I perform are: Closing speaking space, teeth relation at swallowing , teeth relation at rest position, all gear to determine restorative space;
😎 If not TMD present, changes in OVD can be performed with cautious testing with provisionals . If TMD is present, a trial splint at desired new OVD is a must and adjust accordingly until no symptoms by patients ;
πŸ€“ A convo approach with increase in Vertical Dimension of Occlusion (5mm anteriorly) and esthetics and functional crown lengthening (3mm) was performed to achieved this outcome;

🧐 to help reduce the esthetic risk in this case a biotype conversion and increase of gingival volume was necessary at time of implant placement;
πŸ€“ SCTG from 2 different tuberosity sites were harvested and secured in place at implants sites and between the implants to help bulk up the entire span;
πŸ€“ We can see at 6 weeks how the tissue has healed and looking thick and healthy;
🀨 This is the ideal situation for when implant provisionals are fabricated to be able to train the soft tissue and develop the interproximal papilla , societally between 7 and 8 sites;

🧐 Another variable that complicates the equation of ERA is 2 implants Vs. 1 implant to replace a central and a latera;
πŸ€“ Most of the time , 1-implant supported cantilever FDP has been my choice due to esthetics reason to replace interdental papilla with pink porcelain, but few times I have encounter prosthetic complication with the connection at implant level making me wish to have 2 implants instead of 1;
🀨 In this case I decided for 2 implants and augment the soft tissue (thick biotype) to gain enough pink architecture to later scallop it using provisionals;
😎 As you can see, depicted by dotted lines, I have increased incisally the volume of soft tissue, leaving the interdental papilla for developing later;
😬The esthetic zone is not a joke;

β–ͺ️ Yes, with the right diagnosis comes the right treatment. Adults many times have had no ortho treatment or some have had several orthodontic treatment over the years, yet when it comes to full mouth rehabilitation, often times teeth aren’t in the right place or relationship. If that is the case, teeth could be negatively and heavily restored to get them into an adequate esthetics and function;
β–ͺ️ At our office, we make the right call with every patient determining the need to move teeth before rehabilitation, preventing unnecessary removal@of excess tooth structure that could lead to root canals;
β–ͺ️ In this case, patient presented with a severe class II , lack of posterior occlusion (bite) and heavy wear of posterior teeth leading to flare of anterior incisors, unaesthetic and unprotected occlusion;
β–ͺ️ 18 month of orthodontic treatment lead to a more acceptable teeth relationship before Veneer, crowns and implants were made;
