“ Esthetic Compromise “ ?
⁉️ Maybe , papilla between 7 and 8 was difficult to sculpt , even after tuberosity SCTG and long-term provisionals;
🔹 The addition of veneers on the rest of the worn dentition made masking of the lack of papilla inconspicuous at first glance;

“2- Implants rather than 1-Implant cantilever FDP “ ??
🔹Yes, I have done so often the 1-implant cantilever to improve gingival esthetic , but finding unwanted mechanical complications that made me rethink the hard pursuit of pink esthetic;
🔹 Pink porcelain material and technicians skills still faces challenges mimicking that of natural soft tissue making hard to find the great match;
🔹 It then becomes a trade off between minor to non-complications with 2-implants and lack symmetry in soft tissue esthetics Vs. Potential bio mechanical complications with 1-implant supported FDP with better soft tissue symmetry;
🔹 No patient like any potential complication with the treatment , then they should know ahead that gingival symmetry with be a compromise;
🔹 As described before, this patient ERA (esthetic risk assessment)was HIGH as he presented with 2 missing teeth in the esthetic zone with a Medium smile line;
🔹 We tried multiple times to add pink porcelain and being unsuccessful in matching the soft tissue hue and texture. The outcome of no having the pink porcelain, in this case, resulted in a more pleasant senile for both: patient and our team;
🔹 Porcelain veneers were added to the worn dentition to improve overall esthetics;

“ Keeping it simple”??
🔹 Yes, in regards to Occlusion in Full Arch & Material Selection, Zirconia can handle mutually protected occlusion, but if resins teeth are selected , group function or progressive disocclusion will handle better the increase wear f the material overtime;
🔸 Material thickness or Restorative space: when selecting Zirconia as monolithic material a minimum thickness of 13mm is recommended, if no enough space then consider increase the # of implant and the spread in the planing phase;
🔹Layered zirconia should be minimal and confined to the facial, no incisal, of the prosthesis 🤓

“Dual Zirconia Fixed Complete” ?
🔹 Yes, beauty & strength;
🔸 But, it does come with its own challenges , specially when it comes to biology and implants. In this case smoking;
🔹Smoking can create up to 12% complication rate , therefore the way to deal with this is smoking cessation counseling, or just put more number of implants throughout 😝

i-Pad based Photogrammetry??
🔹 Yes. This technology have been released recently by with the name TMarker and I am very excited as the cost is not even close to the expensive PIC or iCam;
🔹 We tried it, design, printed and load same day using these scanbodies and an IPad. Incredible fit, but it was only a PMMA;
🔹 For my final I will want to use this same scan, but also make a jig to verify the accuracy of these scan bodies and system;
🔹 Are you open to the new and less expensive photogrammetry?

Full Zirconia direct to mili-units ?
▫️ Yes, it can be done with a fully digital workflow or analog workflow. My prefer is to use a combined approach using both of them;
▫️ No debonding of ti-basis, therefore less variables to the equation of stability;
