“ Two implants or One Implant Supported Cantilever” ?
▪️ In the past I have supported the one implant-supported cantilever approach as it better replaced the deficiente papilla that occurred when 2 adjacent teeth are removed ;
▪️ But, I have encountered issue with the prosthesis becoming loose, maybe due to third party abutments, or simple as this is the most common complication in single implant-supported prosthesis;
▪️ still 2 implantable supported splinted prosthesis can become a challenge to hygiene and compliance and could bring about biological complication;
▪️ What about individual implant-supported prosthesis ? The challenge is in the esthetic result as it will be impossible to establish a papilla that symmetrical with contralateral side . But, doing the best we can with Pre-prosthetic surgery and gingival augmentation we can make the esthetic outcome more visually pleasant;
▪️ It all comes to the ERA and the smile line to make sure it is a an acceptable compromise. The provisional phase , bottom photo, is the crucial step in assessing the the final outcome by you and the patients and decide on moving forward with final restoration;

“ Can you apply and evaluate the esthetics parameters when looking only intraorally” ??
🔸 This is a continuation post from the case of yesterday ;
▪️ The answer is no, we cannot apply the esthetic parameters when looking inside the mouth only. We must put a face to the teeth to be able to measure anatomical landmarks and set the starting point;
▪️ By looking at the photo above , we would be lost to set the correct position of the teeth in harmony with esthetics and function ;
▪️ This is a typical case of inverted smile when the pre-maxilla has lost bone volume and the maxillary posterior segment have supra-erupted due to lack of mandibular occlusal support. So how do we fix this mess ?
▪️ By putting a face and smile to the photo above we can start gauging where the future maxillary incisors would be as you starting point for the smile design and occlusal plane . From there we just remove what is in our way and add what is missing;
🔹 This is a combination of conventional maxillary denture after extraction and aggressive bone plasty (see photos), opposing a mandibular locator (no hole no screws) fixed complete (see video and photos);
