Harbor Care, formerly The Partnership for Successful Living, has been able to achieve real and significant outcomes within Greater Nashua and NH. Working in partnership has proved invaluable in helping those we serve—thousands of New Hampshire’s most vulnerable community members—realize their own successes. Housing plus integrated care really does equal success. In Greater Nashua, Harbor Care has realized:
-97% decrease in unsheltered homelessness (since 2005).
-3 years with no veterans in long-term, unsheltered homelessness.
-25% reductions in annual overdoses and overdose deaths (the lowest rates since 2015).
-94% decrease in chronic homelessness (since 2005).
-10+ years with no individuals living with HIV/AIDS in long-term, unsheltered homelessness.
-96% viral suppression rates among our HIV/AIDS case management clients.
