I came to Southern New Hampshire in 1978; and after almost 30 years in practice, I love my work more than ever. Plastic surgery is not magic; it is real surgery. While a great deal can frequently be accomplished, you and I must understand each other to develop an effective surgical plan that you wish to carry out. We must work together before, during and after surgery to achieve the best possible outcome. My goal is always the best attainable surgical result for both airway and appearance, with the greatest possible safety. Each member of my staff shares my philosophy. Still, you must understand the limitations of what is possible, collaborate with me to develop a surgical plan, and follow it through to completion. None of us wants an unhappy patient or an imperfect surgical result. We all want things to go "just right," every time. But that would not be surgery, that would be magic.

Pia Melody, whom I have known for more than 20 years and who was a formative influence on my life, passed away last week at 82. She was the founder of the mental health program and trauma workshops at The Meadows in Wickenburg Arizona. I took many of the workshops for my own health and another on the way therapists are trained. Three were with Pia herself. All of that work and the reading that it generated opened the doors for me to understand understanding the world around me and in particular my patients, those who struggled with self-harming behaviors and addictions to plastic surgery. Pia's ideas all came in a rush in a very short period, as I have seen happen with the other genius in my life, Dr. Jack Sheen. She has a legacy not only at The Meadows but through all the therapists that she trained. What was revolutionary about her work was that she recognized that childhood trauma works at the midbrain level; that is the area that has to be accessed and treated. Childhood trauma is not just a cortical, rational problem, and therapies aimed at that area alone may not be fully successful. I will remember her with great affection and respect.

I am I teaching my course on closed rhinoplasty at The Aesthetic Meet 25 on March 22. Instructing with me will be Dr. Ariel Rad. Looking forward to it! Just calculate that this is my 85th national teaching course on rhinoplasty. Each year it is a new course, as I tried to make the concepts and techniques even more clear for the attendees. Our latest research, the first study on the childhoods of plastic surgeons and its effect on their adult health and behaviors was published this week, and Dr. Kenkel, the Editor in Chief, was kind enough to make it his choice article for the month. We are very honored. This was a big study and I hope will inspire other similar work.
