We love partnering with physician offices!

Partnering with TeleMate Health allows Doctors, Physicians Assistants, and Nurse Practitioners to monitor their patients vital signs closely between appointments in order to make sure their blood pressures, blood sugars and oxygen levels are staying within normal limits and interventions aren’t needed.

Our Remote Patient Monitoring services are great for patients that are medium to high risk for hospitalizations, have just had a medication change, or patients that need more support and education on their chronic condition while at home!

So what do we do at TeleMate Health?

The short answer is, we do A LOT, but one of our favorite platforms is Remote Patient Monitoring!

Ever heard of it? The truth is, it’s been around for YEARS but more people need to take advantage of it. Medicare encourages its use because it’s proven to keep people out of the hospital!

So what is it?

RPM uses digital technologies to collect medical and other forms of health data from individuals in their homes and electronically transmits that information securely to health care providers in a different location for assessment and recommendations.

So, we provide people with vital sign equipment (blood pressure monitors, glucometers, pulse oximeters, scales, spirometers) AND a personal nurse that is directly connected to that piece of equipment. That means every time you take a reading with your equipment, the smart hub pictured below 👇 sends that reading safely and securely to one of our nurses! The nurse will call you to check on you if readings are too high or too low or, you are able to call the nurse!

But wait….there’s more….

When necessary, the nurse will contact your primary care provider or specialist to inform them of readings that trend outside of parameters AND your doctors 🥼 get your readings faxed to them every month so they stay informed on how you are doing!

We’ve found that being this extra layer of support for people and bridging the gaps between the patient and physicians have been extremely beneficial.

RPM is covered 100% by Medicare with a secondary, all Medicare Advantage plans and most commercial plans. You pay nothing 👏

We want to thank our friend Steve for helping pass out flyers to the Trevecca residents for Monday’s meet n’ greet with one of our favorite Nurse Practitioners Linda Byrd! Linda has been providing amazing telehealth services through our kiosk to the residents and started once a month in person visits in order to bring the best healthcare to them! The residents are excited to meet her in person and it should be a great day!

Our company is very passionate about giving back to the community and today, we were able to do a little of that. We want to thank our great helper Trey Michael for helping us put together and deliver these totes full of goodies to some special people today.

We have officially been handed the keys 🔑 to the Trevecca Tower clinic. They love what we’re doing so much, they want us and our kiosk to be the providers in the clinic. The amount of gratitude 🙏 these residents have and the ability to provide them with healthcare and behavioral health support that they haven’t had in years for a multitude of reasons is a true blessing.

We are exhausted 😴 from a busy day but you won’t find a team of people anywhere that will work as hard as we do to make sure people are taken care of. Our business can help people with their healthcare in a lot of ways so if we can help you or your family member, let us know.

Busy day today at Trevecca Towers getting residents set up on remote patient monitoring! They are SO thankful for the extra help with their healthcare and we have truly come to love these people!

With Remote Patient Monitoring we provide patients with vital sign equipment in their homes so they can take their blood pressure, weight, blood sugar or oxygen daily and that reading is transmitted straight to our nurses for close monitoring and communication to the patient if necessary.
