This patient came to Dr. Biesman to discuss concerns about hollowing of her upper eyelid area, known as the superior sulcus. A deep superior sulcus refers to a loss of youthful fullness beneath the orbital bone but above the eyelid platform, which can create unwanted shadowing and a sunken appearance.

To correct this area, Dr. Biesman used strategically placed filler, to gently fill along the base of the orbital rim, camouflaging the hollowness without adding bulk or puffiness to the eyelid. You can see how the result softens the contour, supports the upper lid, and gives the entire eye area a more refreshed look.

Acne scarring can have a lasting impact on how someone feels about their skin, and we understand how personal and emotional that journey can be. For many of our patients, it's something they’ve felt embarrassed or insecure about for years. While major improvement is absolutely possible, it’s important to know that this process is a marathon, not a sprint.

This patient’s transformation didn’t happen overnight. Over the course of several years she followed a customized treatment plan that included multiple subcisions, scar excisions, and laser resurfacing procedures to achieve significantly smoother, less pitted skin texture.

Subcision releases the fibrous bands pulling a scar down from beneath the surface, allowing the skin to gradually lift and remodel. This patient underwent upper eyelid blepharoplasty and endoscopic brow contouring—a highly effective and popular combination that addresses both eyelid heaviness and brow descent.

During the surgery, Dr. Biesman modifies the upper eyelid and recontours the brows using small incisions hidden in the hairline. In the after photo, you can see that the outer half of the upper eyelids, which were previously weighed down and caused the crease to appear uneven, are now lifted, creating a defined, balanced contour across the lid. The brow position has also been subtly raised, which helps to open the entire eye area.

This patient was treated with a series of hyperdilute Radiesse to improve skin texture and firmness in the abdomen.​ While Radiesse is a filler ​commonly used to restore volume in the face and hands, ​it can also be carefully diluted and injected just beneath the skin to stimulate natural collagen and elastin production. Over time, ​i​t helps improve overall skin quality​ and elasticity without adding volume.

In the after photo, you can see a noticeable smoothing of the crepey texture and increased skin tightness after treatment. Results develop gradually as​ the collagen rebuilds ​in the weeks following treatment and typically last 12 to 18 months, though it varies depending on the patient. Multiple sessions are typically necessary for best outcomes.

This patient underwent upper eyelid ​surgery to remove excess skin, ptosis repair to ​address the weakening muscles and elevate the upper eyelids, and an endoscopic brow lift to reposition and stabilize the brows.

Ptosis of the eyelids means that the eyelid covers part of the pupil, almost like a window shade that is pulled down. This can produce a tired, angry, or ​asymmetrical appearance. By tightening the muscles that elevate the eyelid, Dr. Biesman can restore proper eyelid aesthetics and help the eyes appear more alert. Performed at the same time, endoscopic brow lifting addresses eyebrow descent and shape, repositioning the brows for a more youthful appearance and provides a long-term result.

Dr. Brian Biesman specializes in cosmetic and reconstructive eyelid and facial surgery and minimally invasive rejuvenation techniques including injectables (Botox and fillers) and technology-based solutions (laser skin resurfacing, and related techniques), and medical grade skin care. In addition Dr. Biesman is a nationally and internationally recognized authority on noninvasive body contouring techniques such as CoolSculpting® for the elimination of unwanted fat. Dr. Biesman serves as a Clinical Assistant Professor at Vanderbilt University Medical Center, Nashville, TN, where he holds appointments in the divisions of Ophthalmology, Dermatology and Otolaryngology-Head and Neck Surgery.

Dr. Biesman’s office is conveniently located on the corner of 23rd Avenue North and Charlotte Avenue, with ample free parking available in a structure attached to the building via a covered walkway. Valet service and self-parking are available. Dr. Biesman’s Suite is in the Centennial Professional Plaza Building, a free-standing four-story building across the street from the Emergency Room Entrance of Centennial Medical Center.
