Cosmetic concerns are common when a diastasis recti (DRA) is present.

The overarching functional goal of DRA rehab is to create an abdominal wall that can produce tension, allow for load transfer into the lower extremities, and to manage pressures which all can happen without true “closure” of the gap.

Factors that can be helpful to consider when rehabilitating a diastasis recti (DRA):

-Pelvic & Ribcage- the pelvis and ribcage are designed to be dynamic.
A ribcage and pelvis that stay more flared out might prevent approximation of a DRA.

-Breathing: A breath should be 360 degrees. If someone tends towards belly breathing as a dominant strategy, fascia could sustain overstretching preventing approximation.

-Pelvic Floor: A tight pelvic floor will resist the intraabdominal pressure coming down, so the bulk of pressure will go through the weak fascia in the front, preventing approximation.

-Appropriate loading: the muscles and the connective tissue have to be challenged enough to increase in tensile strength and in order for remodeling to occur.

-Compensations: When certain exercises intended for the transverse abdominis are too difficult, often the re**us abdominis will become more dominant reducing the ability for approximation.

This could also apply to compensations that happen when we bypass the abs as we move, reducing the stimulation needed for healing (i.e. not rotating well through the thorax when loading or unloading dishes).

-Structural Tension: Tight fascia that integrates with the abdominal fascia creating an opposing pull

-Ab wall balance: The influence of both tight and weak ab wall muscles on the DRA needs to be considered to assist in approximation.

Pelvic PTs/OTs can have vastly different education and different comfort levels assessing and treating other joints, or in the awareness of how other joints influence hip and pelvic health.

Similarly, some orthopedic physical therapist”s may have knowledge gaps when it comes to hip and pelvis anatomy .

For chronic or complex hip pain, back pain, tailbone pain and some knee pain an understanding of the complex biomechanics of the whole body may be needed for lasting relief.
