The Center for Case Management

The Center for Case Management Our core business is customized consultation and education for managing clinical care toward measurable outcomes.

As the innovators of outcome-based clinical paths (CareMap™ tools) and clinical case management, CCM constantly expands those and other methods for achieving cost/quality outcomes, including analyses of case type level data and case management performance indicators for both payers and providers. CCM provides solutions and implementation strategies to meet the unique needs of clients as they seek ways to meet mission and margin targets across the continuum.

As a nurse scientist, Sheree Loftus, PhD, MSN, APRN-BC, helped advance Parkinson’s research, deep brain stimulation, and evidence showing that exercise improves balance and cognition. She also fought to ensure nurses were recognized as full members of research teams, long before it was standard practice. Previously, only the names of doctors and PhDs were listed as authors on published papers.

Because of trailblazers like Sheree, nurses are now well-known as essential contributors to scientific discovery and innovation, helping improve health outcomes for all.

“People don’t know nursing research exists, but we make a difference,” Sheree said. “The Power of Nurses™ is guiding patients through the healthcare system – whether that’s with clinical expertise, compassion, or research.”

As Social Work Month comes to a close, we proudly celebrate the extraordinary impact of social workers—especially those who have helped shape the heart of our organization.

Today, we’re honored to spotlight one of our original social work consultants, Matt Boettcher, and the legacy he continues to build 💙

When asked what social work has taught him, he said:

“Surgeons save lives—but that’s not enough. My role is to make the life they saved worth living. That’s what social work brings to healthcare.”

We couldn’t say it better.

To every social worker—thank you for the care, compassion, and hope you bring to people every day. We appreciate you 💙

It’s Social Work Month 💙

We’re spotlighting one of our amazing OG social work consultants, Shawna Kates!

Here’s what she shared:

“Social Work has been more than a profession for me—it’s been a calling. A mission. The more I practiced, the more I was motivated. What I learned was that people have greater strength, positivity, and resilience than we often presume.”

We’re grateful for the heart and dedication social workers bring every day! 👏

In recognition of Women's History Month, we’re proud to highlight the leadership behind our organization.

Meet our President & CEO, Bonnie Geld, MSW.

Bonnie brings more than two decades of leadership in healthcare case management. Her work has focused on designing longitudinal case management programs that support patients across the continuum of care, strengthening transition planning, improving processes, and building collaborative teams that successfully integrate both social work and RN expertise.

Uplift. Defend. Transform.

This Social Work Month, we recognize the extraordinary impact social workers have across healthcare and our communities.

Social workers uplift individuals and families in their most vulnerable moments, defend dignity and access within complex systems, and transform lives through advocacy, compassion, and expertise.

In healthcare case management, their work is essential—connecting people to the care, resources, and support they need to move forward.

Today and every day, we thank social workers for the profound difference they make.
