ILS Specialist holds primary responsibility for areas of health care/nutrition community and household management for the client. The ILS Specialist is involved directly in all assessments, planning, and implementation of services and supports. The ILS Specialist will assist in direct training, follow up, coaching with service staff, and keeping the Nurse and Supervisor informed as needed, helping the client in maintaining good health; and other specific supports, as described in Individualized Service/ Support Plans.

The ILS Specialist has the responsibility further to coordinate a variety of services including but not limited to: finances, recreation, health services, family involvement, educational/vocational programs, and all other supports/services necessary to ensure a good quality of life. The ILS Specialist is responsible for assisting the Nurse and Supervisor in maintaining consistency in the provision of supports to the client and in communicating with agency staff and team members.

EXPECTATION:
PCA/CFSS/DSP staff would assist the client with the following ADLs:
Dressing
Bathing
Eating
Transfers
Mobility
Positioning
Toileting
Instrumental activities of daily living (IADLs)
Accompany to medical appointments
Accompany to participate in the community
Assist with paying bills
Communicate by telephone and other media
Complete household tasks integral to the PCA services, such as:
Plan and prepare meals
Shop for food, clothing, and other essential items
health and hygiene
Grooming

May observe and provide redirection to the recipient for episodes of behavior needing redirection as identified in the care plan.

Supporting individuals using positive support strategies in order to help them develop, achieve goals and advocate for themselves

The Home Care Program Manager will:

• Take Client Intake prior to admission

• Orient, train, and evaluate individual PCA staff

• Oversee the delivery of PCA services while working with the QP Nurse

The Home Care Program Manager must train, orient, and schedule PCAs on the needs of the person at the location where PCA services are being delivered.

The Home Care Program Manager must work with the QP/Nurse to evaluate regularly scheduled PCAs within the first 14 days of providing services to the recipient, at the location where PCA services are being delivered.

The Home Care Program Manager must work with the Nurse to use professional judgment to determine the number of visits needed to ensure quality care.

The Home Care Program Manager must schedule a monthly call check on all clients

The home Care Program Manager will manage all affairs of the office

Must be willing to cover shift pending shift coverage by new staff

Must be professional when communicating with staff or client

Must be willing to undergo required training

Must be willing to undergo a background check
