Advancing Health IT.
CHQR badges recognize Health Center Program awardees and look-alikes that have made notable achievements in the areas of Access, clinical quality, health outcomes, and health information technology. These badges are awarded annually after HRSA, the Bureau of Primary Care, reviews health centers’ performance data from the previous year.

Every August, the National Association of Community Health Centers (NACHC) sponsors National Health Center Week (NHCW) to celebrate and increase awareness of America’s 1,400 Community Health Centers (CHCs). National Health Center Week is an opportunity to highlight the commitment and passion of Community Health Center staff, board members, and supporters who make it possible to provide quality, comprehensive healthcare services to more than 31.5 million patients across nearly 15,000 communities annually.

Community Health Centers serve as a beacon of strength, service, and care in their communities. In moments of pain and loss, they offer support and love. In moments of triumph, they offer hope and a vision for the future. This year’s National Health Center Week theme focuses on empowering communities by connecting them with health centers and elected officials from all levels of government. Mental Health Awareness Month
May is Mental Health Awareness Month. Since its inception in 1949, Mental Health Awareness Month has been a cornerstone of addressing the challenges faced by millions of Americans living with mental health conditions. Throughout the month, National Alliance on Mental Illness (NAMI) actively participates in this national movement, dedicated to eradicating stigma, extending support, fostering public education and advocating for policies that prioritize the well-being of individuals and families affected by mental illness. As we close the month, please remember to discuss all concerns you may have with your provider. In case of an emergency, dial 911.
Our staff wore green to recognize Mental Health Awareness Month. The Patient & Staff Engagement Committee provided soothing tea, relaxation gift bags and information regarding mental health services.

The Eric B. Chandler Health Center is a comprehensive, family-oriented community health center, jointly operated by the Robert Wood Johnson Medical School and Eric B. Chandler Community Board, Inc.
The Center, opened in October 1987, is dedicated to the memory of Dr. Eric B. Chandler, an active and respected community leader recognized for his work in the National Association for the Advancement of Colored People and a member of the Board of Directors of the Urban League of Greater New Brunswick.
Primary care services available at Chandler include: family medicine; internal medicine; infectious disease (HIV counseling, testing, and treatment); pediatrics (including EPSDT and child well-care); obstetrics and gynecology; podiatry; family planning; and dentistry (including preventive, restoration, and curative). Other services of the center include social services, community outreach, nutrition, and health education. Laboratory services are offered as well.
Services of the center are provided by appointment. A variety of insurances are accepted including Medicaid, Medicare, and various types of managed care and private insurance plans. The center also offers a sliding fee scale, which is updated annually based upon the Federal Poverty guidelines. The Eric B. Chandler Health Center provides a telephone on-call rotation of physicians available 24 hours a day.
The Eric B. Chandler Health Center's Mission is:
· To provide high quality ambulatory health care services that are culturally effective, accessible, affordable.
· To promote a healthy lifestyle and educate patients to take responsibility for and participate in their health and health care decisions.
· To serve as a community resource for health and social services.
· To provide high quality educational opportunities for health professionals who train at the Center.
