The Nursing Home Transition (NHT) Program at DON Services is excited to share that we’ve successfully completed our 26th transition this year! 🎉

The NHT Program was developed to empower and assist individuals who wish to move from a nursing facility back into a home of their choice in the community. When given the option, most people express a strong desire to age in place, and our program helps make that possible.

Through NHT, we work to remove barriers within service systems, ensuring that individuals receive the supports they need in the setting they choose. Participants and their families are fully informed about long-term service options, including Home and Community-Based Services, so they can make confident, informed decisions about their future.

👏 Congratulations to our NHT team and participants on another successful transition, creating opportunities for independence, dignity, and community living.
