JOB OPPORTUNITY! We are looking for an experienced service coordinator in Rockland County. Master's degree is preffered, and experience is required. If interested please contact Jay at 845-826-6090.

The service coordinator assists the prospective participant to become a waiver participant and coordinates and monitors the provision of all services in the service plan. Services may include medicaid state plan services, non-medicaid federal, state and locally funded services, as well as educational, vocational, social, and medical services. The goal is to increase the participant's independence, productivity, and integration into the community while maintaining the health and welfare of the individual.

Responsibilities:

Responsible to assist the waiver participant in the development of the individualized service plan
Responsible to assist the waiver participant in implementing the plan
Support and encourage the waiver participant to complete the plan of protective oversight
Responsible forthe timely submission of service plans and for ongoing monitoring of all services
Asses service plans and initate and review the 6 month reassesment of the waiver participant's level of care
Scheduling team meetings
Responsible for mantaining thorough and appropiate documentation of all visits
