Periorbital aging is nuanced. Volume loss, tissue descent, and changes in skin quality all contribute to tired-looking eyes.
Autologous fat grafting (fat transfer under eyes), when used thoughtfully, can be a powerful adjunct. Your own tissue, refined and precisely placed, acts as a living filler to restore volume, soften hollowing, and improve under-eye skin quality and light reflection.
In my practice, under-eye fat transfer is most often combined with lower blepharoplasty, brow lift, or midface lift to create a smoother lid–cheek transition and more natural rejuvenation. While it can be performed alone in select patients, it is not a substitute for structural correction.
Like everything in oculoplastic surgery, less is more. This is a highly technique-sensitive procedure with little margin for error. A significant part of my work involves revising prior or overdone results, reinforcing the importance of precision, experience, and aesthetic judgment.
When done well, you won’t see it.
You’ll just look rested.
The art is knowing when—and how much—is enough.
