Delaune's Pharmacy and Home Medical is excited to announce the launch of a new Community Health Worker (CHW) initiative, designed to support community members with critical resources and education to improve their health. This new program reflects our commitment to improving health outcomes and supporting our local community’s health and wellness.
The Pharmacy Technician Community Health Worker is the vital link between healthcare providers and the community, offering education, resources, and advocacy to enhance the health and well-being of their community. Heather Breaux and Andrea Bourque have recently earned a Community Health Worker Training Certificate.
Delaune's Pharmacy is excited to expand our services and better address the needs of our patients.
Coordination: Heather and our staff will provide one-on-one consultations to help individuals navigate their health challenges, access resources, and understand their medications better.
ď‚· Health Education: Through workshops and community outreach, our CHW will
educate residents on important health topics, including chronic disease
management, nutrition, and preventive health measures.
ď‚· Increased Access to Resources: Our CHW will assist patients in identifying
and accessing local health services, insurance options, and community programs
to help our patients get the support they need.
