Nurse Practitioner Position Summary:
Provide comprehensive family practice services to patients across the lifespan, including annual wellness visits, preventive screenings, chronic disease management, acute care, and health education. Work collaboratively with clinical team members to promote continuity of care and improved patient outcomes.
Responsibilities:
Nurse practitioner led practice dedicated to patient centered care.
Provide comprehensive primary care services to pediatric, adult, and geriatric patients.
Perform physical examinations, diagnose acute and chronic conditions, and develop appropriate treatment plans.
Order, perform, and interpret diagnostic tests and laboratory results.
Prescribe medications and treatments in accordance with state regulations and clinical guidelines.
Deliver preventive care services and manage chronic health conditions.
Educate patients and families on health maintenance, disease prevention, and wellness strategies.
Maintain accurate, timely documentation in the electronic medical record (EMR).
Athenahealth EMR
Collaborate with providers and the care team to ensure coordinated and continuous patient care.

Qualifications:
Licensed Nurse Practitioner in New Hampshire
Strong clinical decision-making and communication skills.
Ability to work independently and as part of a team.
Experience in primary care or family medicine preferred.
