Life Long Care of New London, PLLC Life Long Care opened in 2003 and has been serving the New London area since! We have a wonderful staff of skilled nurses and APRN's. Become part of our family.

Our Practice
We are a primary care family practice of nurse practitioners, a nurse, and medical assistants. Here are a few examples of what we do every day:
Annual physical exams
DOT physicals
Blood tests
Immunizations
Family planning
Nutritional counseling
Management of complicated and/or chronic medical issues
Referrals to specialists

Life Long Care is also a recognized Patient-Centered Medical Home! The Patient-Centered Medical Home is a team-based health care concept. We coordinate all of your medical care, making referrals to specialists when needed, and making sure routine labs and other tests are done. The goal of this type of health care model is to maximize healthcare outcomes, which translates into improved, more comprehensive care for you!
