Have you heard about of the ? Achalasia is a rare disease (1:100.000)it is characterized by the failure of relaxation of the lower esophageal sphincter (LES), accompanied by a loss of peristalsis in the distal esophagus. Treatment of achalasia is aimed at decreasing the resting pressure in the LES to allow passage of ingested material. Achalasia can be treated with pneumatic dilatation (balloon), botulinum toxin injection, endoscopic (aka ) or surgical myotomy ( ). Laparoscopic Heller myotomy is the most commonly performed surgical myotomy procedure. At LSU Department of Surgery we perform this procedure using . Robotic Heller myotomy has shown to have less complications (ie intraoperative perforations). We also perform endoscopic myotomy aka POEM procedure. We will discuss the POEM in the next post...⏳

What is the difference between BPD-DS and SADI-S for weight loss surgery? BPD-DS (biliopancreatic diversion with duodenal switch) and SADI-S (single anastomosis duodenal ileal bypass with sleeve gastrectomy) are two surgical procedures used for . They usually result in significant weight loss even higher than and . In addition, they are great for improvement of associated medical conditions such as . The BPD-DS (picture left) includes a sleeve gastrectomy + 2 anastomosis (intestinal connections). The classical DS-BPD is a technically more challenging operation. SADI is a less complex surgery, since there is only one intestinal connection, producing almost the same weight loss and metabolic results. SADI-S has also less reported nutritional complications. They both can be used as a primary (first) surgery for weight loss or revisional surgery.

And ASMBS 2024 is behind us. Great Congress! Lot of conversations about the weight loss revolution/new medications for treatment. My take home message is; we have three highly effective treatments for obesity. We need to embrace all the therapeutic options available and try to match patients with the best treatment based on individual characteristics. Research demonstrates that these treatments, when used under specialized care, are synergistic. We have to implement a contiuum of care that tracks patients overtime since there is today no cure for

We usually get asked what is the SADIS or duodenal switch surgery
Single or one anastomosis duodenal switch or SADIS. Is the most recent Bariatric surgery endorsed by ASMBS - The American Society for Metabolic and Bariatric Surgery. The operation includes the sleeve gastrectomy, then the first part of the small intestine is divided just after the stomach/sleeve and finally a loop of intestine is measured several feet from its end and is then connected to the stomach. This is the only intestinal connection performed in this procedure. The operation is simpler and faster to perform (one intestinal connection). Same number of incisions as sleeve/gastric bypass and patients usually go home after overnight stay. Very effective for weight loss in patients with more severe obesity (higher BMI) and remission of type 2 diabetes, and less nutritional complications.
