Some of our patients have asked what they can do when their insurance doesn’t currently list Kisha as in-network at her new practice.
One of our patients took the initiative to draft a letter that can be sent directly to your insurance provider requesting that they credential Kisha at Kisha Cares. We are sharing it here in case anyone else would like to use it as a template. You are welcome to copy, edit, and send it to your insurance company if you would like to advocate for continuity of care.

To Whom It May Concern,
I am writing to formally lodge a complaint regarding Aetna’s refusal to allow my primary care provider, Kisha Howard-Schramm, to remain in-network following her transition to her own private practice.
For some time, I have received excellent care from Kisha. She was previously an in-network provider with Aetna while practicing at New Windsor Medical. However, since opening her independent private office at 3141 Route 9W, New Windsor, NY 12553, she has reportedly been informed that Aetna will not credential her as an independent provider solely because she is a Nurse Practitioner.
I would like to bring to your attention that under the New York State Nurse Practitioner Modernization Act (NPMA), experienced Nurse Practitioners with more than 3,600 hours of practice are legally authorized to practice independently without a written collaborative agreement with a physician. By denying her credentialing based on her status as an NP in private practice, Aetna may be creating an artificial administrative barrier that the NPMA was specifically designed to eliminate to ensure patient access to care.
As a loyal Aetna member, I am requesting that you review Kisha Howard-Schramm’s credentialing status at Kisha Cares immediately. Continuity of care is vital to my health, and I should not be forced to find a new provider when my current, qualified provider is legally entitled to operate her own practice in this state.
Please provide a written explanation for the denial of her independent credentialing or an update on her status within 30 days.
