Peter K. Stafford Health Care Consulting, is devoted to assisting religious/faith organizations, small health care companies, or non profit agencies, direct their individual clients to large Health Home programs, that could provide more services for their clients.
For instance, a small non profit might not have the resources to help the homeless population find housing , or employment. Furthermore, organizations, that have alcohol, or substance abuse patients, clients suffering from mental health issues, or former incarcerated individuals, military veterans, or HIV patients, might need access to health care, housing, and vocational training.
I worked out of the New York City Health and Hospitals Corporation's main corporate office in Manhattan, in New York City, for two years where i was a Community Outreach Specialists, where I had enrolled Medicaid eligible patients in the New York State Medicaid program. I dealt with patients that suffered chronic medical issues, mental health issues, homeless issues, HIV patients, military veterans, alcohol and substance abuse patients. I would get lists from many of the eligible health insurers of the state, I was also required to make cold calls off of eligible clients, I also visited patients in their homes and hospital rooms. Furthermore, I interviewed psychiatric patients at the units of Bellevue, Harlem Hospital, Metropolitan Hospital, Montefiore, and all city hospitals, the Psychitric unit at Randall's Island.
