As a PT for 17+ years, who is also a Board Certified Specialist in Orthopedics; I’ve seen how much what we were taught around shoulder pain has evolved.

At points, fixating and demonizing patient posture , Leg discrepancies and scapular positioning…but over time, research has challenged a lot of what we once thought was black and white. ACL rehab and management being one such example.

What continues to stand out more and more is this: movement matters. It’s rarely linear, rarely pretty, and rehab doesn’t always fit neatly into boxes. But at the end of the day, we want our patients to move, build confidence, and get back to doing what matters to them.

I’m really grateful I attended this workshop (Dr Ivan) because it reinforced something I think we as PTs can sometimes forget… we tend to overcomplicate things. When in essence, sometimes we just need to keep it simple.

One of my favorite takeaways from finally meeting Adam Meakins—someone whose content I’ve followed online for quite some time—was hearing the phrase he kept coming back to: “It depends.” Because in rehab, is anything ever truly perfect, absolute, or one-size-fits-all? Usually… it depends.

I also appreciate clinicians who continue to challenge old thinking and make space for updated research, especially in a world where a large following can sometimes make information feel untouchable or “gold.” Growth in our profession requires staying curious, open-minded, and willing to learn, unlearn, and rethink.

Grateful for the learning, the perspective, and finally getting to connect in person. Always a student first. 🙏🏽

Three years into being a physical therapist, I had a patient come to me through direct access for what seemed like pretty straightforward low back pain.

Female patient. Late 30s, early 40s.

Her primary complaint? Pain around L1/L2.

She described it as an intermittent, dull ache. No pain radiating into the legs. No numbness. No tingling.

Aggravated by sitting and lifting.

At first glance? Sounds mechanical, right?

But then the systems review changed everything.

She told me the pain seemed to come on suddenly…..

No prior history of low back pain.

She was taking naproxen…

…but it wasn’t helping.

Then she mentioned her pain was significantly worse at night.

It was affecting her sleep.

Then came the biggest red flags:

Unexplained weight loss.

Night sweats.

And a previous history of breast cancer.

Her hip mobility was normal.

Her lumbar exam wasn’t particularly remarkable outside of pain.

And that’s when I knew this wasn’t a patient I needed to “fix.”

This was a patient who needed further medical workup immediately.

I referred her back to her oncologist.

She never returned to physical therapy.

Later I found out her breast cancer had metastasized to her lumbar spine…

And this is why systems review matters.

Not every low back pain patient needs manual therapy.

Not every low back pain patient needs exercises.

Sometimes your greatest clinical skill is knowing when NOT to treat.

As PTs, we’re movement experts.

But we also need to be excellent at recognizing red flags.

That can literally save someone’s life.

Stop shaming Women!

Pelvic pain is widely underreported , under diagnosed and under treated and many suffer in silence because of shame.

Pelvic pain for many women means:
• pain with in*******se
• inability to tolerate pelvic exams
• urinary urgency/leakage
• constipation
• pain after childbirth
• hip/back pain
• difficulty relaxing pelvic floor muscles

Pelvic Pain is a medical diagnosis and needs to be addressed.

Park Slope

6 years ago i started my business from my car—- with a table and bag of bands. Offering mostly manual therapy. Driving to suffolk county from my than residence in new jersey.

Later moved into a massage studio in Fort Greeen where was so gracious to allow me to rent a room and i would see clients by appointment only

Later moving to a gym where i grew exponentially for 2 years downtown brooklyn however the gym closed due to low census. The city still reeling from COVID i later settled in Park Slope —on the corner of Union and 8th subleasing but here we are 6 years later.

Thank you to my amazing family, all of my friends and loved ones who cheer me on —the Orthos who sent patients, the run clubs, for taking me on as a client.
