GERD Management Step by Step.

πŸ”Ή Step 1: Lifestyle Changes (First Line for Everyone)
Before starting medicines, simple daily habits can significantly reduce reflux:

Weight loss if overweight

Avoid trigger foods (spicy, fatty, chocolate, caffeine, alcohol)

Stop smoking

Avoid late-night meals (no food 2–3 hours before sleep)

Raise the head of the bed for night symptoms

πŸ‘‰ Many patients improve without any medication.

πŸ”Ή Step 2: Proton Pump Inhibitors (PPIs) – Most Effective Treatment
If lifestyle changes fail:

Start PPIs (e.g., omeprazole, pantoprazole)

They:

Reduce acid production

Heal oesophagitis

Relieve symptoms effectively

⚠️ Important:

Symptoms often return when PPIs are stopped

Some patients need long-term or lifelong PPIs, but always at the lowest effective dose

πŸ”Ή Step 3: Special Situations – Add Other Medicines
🟠 If motility problems are prominent (bloating, early fullness):
Prokinetics help stomach empty faster

Metoclopramide

Domperidone (not available in the USA)

🟠 Mild symptoms only:
Antacids & alginates β†’ quick symptom relief

Hβ‚‚-blockers (e.g., famotidine) β†’ reduce symptoms but do NOT heal oesophagitis

🚫 H. pylori eradication does NOT improve GERD symptoms

πŸ”Ή Step 4: Understanding Long-Term PPI Risks (Explain Clearly, No Fear)
Long-term PPI use may cause:

Acid rebound when stopped suddenly

Reduced absorption of:

Iron

Vitamin B12

Magnesium

Increased risk of gut infections:

Salmonella

Campylobacter

Clostridioides difficile

Possible association with:

Microscopic colitis

Acute interstitial nephritis

Changes in gut microbiota (clinical impact unclear)
