In November 2025, the FDA removed the black box warning that had been on hormone replacement therapy for over 20 years.

That warning — based on misunderstood data from a 2002 study — warned of elevated risks of stroke, blood clots, dementia, and breast cancer. For two decades it shaped how doctors prescribed and how women thought about their own care.

The removal doesn't mean HRT is risk-free. It means care can finally be individualized. Estrogen protects bones, urogenital health, and potentially heart and brain function. That conversation was suppressed for a long time.
