When was the last time you talked to your loved ones about your healthcare wishes? On this National Healthcare Decisions Day, consider filling out an Advance Directive.

These forms allow you to appoint someone you trust to make healthcare decisions for you, just in case you become unable to communicate or lose the ability to make decisions yourself.

Each state has its own set of Advance Directive forms, which you can find online. It's a good idea to submit your Advance Directive forms to your healthcare provider so that they have your wishes on record.
