Lumbar Epidural Steroid Injections for Low Back Pain and Sciatica

Epidural steroid injections (ESIs) are a common treatment option for many forms of lower back pain and leg pain. 

The goals of an epidural steroid injection are to:
* Control pain by reducing inflammation in and around the nerve roots
* Improve mobility and function in the lower back and legs
* Allow the patient to participate in and make progress with a comprehensive physical therapy and rehabilitation program

Potential Benefits of Epidural Steroid Injections
* Reduce nerve pain and inflammation. Steroids decrease the production of inflammatory chemicals and reduce the sensitivity of nerve fibers to pain, generating fewer pain signals.
* Limit oral medication. Pain relief from these injections may help limit or eliminate the need for oral medications,some of which may have side effects when taken long-term.
* Continue or re-engage in physical therapy. This injection may provide sufficient pain relief to allow a patient to progress with a rehabilitative physical therapy program.
* Postpone surgery. Pain relief experienced from lumbar epidural steroid injections may help postpone surgery, and if physical therapy is effective, it may eliminate the need for surgical intervention.

Genicular nerve radiofrequency ablation is new and innovative treatment option has the capacity to decrease pain and improve function and quality of life in patients with Osterarthritis.

Chronic osteoarthritis pain of the knee is often not effectively managed with prescribed medications.

Although surgery is generally effective for patients with advanced disease, some older individuals with other chronic conditions may not be appropriate surgical candidates. In addition some patients do not wish to consider surgery and prefer nonsurgical options. In these patients, radiofrequency ablation of the genicular nerves might be a successful alternative to surgery. This procedure is based on the theory that cutting the nerve supply to a painful structure may alleviate pain and restore function.

During this in-office procedure, the physician uses a radiofrequency device to heat up and disrupt the genicular nerves. These are the sensory nerves that transmit pain signals from the knee to the brain. Disrupting these pathways can provide long term relief from knee pain.

Chronic/Acute neck with pain radiating down the arm?

You may have a herniated disc that is pinching and irritating a nerve root 😖 .  A cervical epidural steroid injection 💉 can decrease the inflammation around the nerve, disc and dura and significantly improve pain and function 😃. The injection serves to facilitate improvement in pain and function.

🎯Fluoroscopy, a type of x-ray, will be used to ensure the safe and proper position of the needle.  A dye will also be injected to make sure the medicine will go into the correct spot.  Once the needle is in the correct location, an anesthetic and a steroid will be injected.

After this procedure the patient just gets up off the table and goes home🏃🏽‍♀️. It's a safe in-office procedure for chronic or acute neck pain that's radiating.

Radiofrequency ablation (RFA) is a procedure used to reduce pain. An electrical current produced by a radio wave is used to heat up a small area of nerve tissue, thereby decreasing pain signals from that specific area.

How is radiofrequency ablation done?
You’ll lie on your stomach or back on an x-ray table.
The doctor will numb an area of your skin with a local anesthetic. Then, he or she will:
* Insert a thin needle into the area where you feel pain; an x-ray can help your doctor pinpoint the exact area
* Insert a microelectrode through the needle; your doctor will ask if you feel a tingling sensation; this helps your doctor identify the right area for treatment
* Send a small radiofrequency current through the electrode to heat your nerve tissue

Which Conditions Are Treated With Radiofrequency Ablation?
RFA can be used to help patients with chronic (long-lasting) low-back and neck pain and pain related to the degeneration of joints from arthritis that does not respond to other treatment, such as pain medication and physical therapy.

How Long Does Pain Relief From Radiofrequency Ablation Last?
The degree of pain relief varies, depending on the cause and location of the pain. Pain relief from RFA can last from six to 12 months and in some cases, relief can last for years. More than 70% of patients treated with RFA experience pain relief.

Is Radiofrequency Ablation Safe?
RFA has proven to be a safe and effective way to treat some forms of pain. It also is generally well-tolerated, with very few associated complications. There is a slight risk of infection and bleeding at the insertion site.

What happens after the procedure?
You can continue your regular diet and medications immediately, but do not drive or do any rigorous activity for 24 hours after the procedure. Take it easy. You can return to your normal activities the next day.
