# # The 5 Movement Systems: Why Your Pain Might Not Be Where You Think

Have you been treating the same body part for months with little progress? Your pain might be coming from somewhere else entirely.

Understanding these five interconnected movement systems might explain why traditional treatments haven’t worked:

🔴 **Core System** - Your stability center that supports all movement

🟠 **Longitudinal System** - Connects feet to back, powers walking and running

🟢 **Lateral System** - Controls side-to-side balance and stability

🔵 **Posterior Oblique System** - Diagonal back connections for rotation and power

🟣 **Anterior Oblique System** - Front diagonal connections for coordinated movement

The key insight? Your back pain might come from your core system, not your back itself. Knee issues might originate in your longitudinal system rather than the knee.

This systems approach explains why treating only where it hurts often doesn’t solve persistent pain.

Still suffering from back pain? 😫Check out ergonomic chairs recommended by occupational therapists! As an occupational therapist, I assess patients with shoulder, neck, and lower back muscle tension by understanding their office environment and lifestyle. This helps address root causes and improve symptoms.
For 8-hour workdays, a high-quality ergonomic office chair promotes correct posture. Here are some key elements to consider when selecting an ergonomic chair:
1️⃣Seat Height: Easily adjustable (16-21 inches) for feet flat on the floor, thighs horizontal, and arms aligned with the desk.
2️⃣Seat Width and Depth: Comfortable support (17-20 inches wide) with depth allowing 2-4 inches between knees and seat. Adjustable tilt desired.
3️⃣Lumbar Support: Vital for lower back; ergonomic chairs should offer adjustable lumbar support (height and depth) to maintain the inward curve of the lower back.

Now this is not what I usually do everyday but during problem solving during the session and following the general principles of treating the musculoskeletal system, the problem resolves.

A patient of mine came in with a complaint on numbness of upper lip that’s affecting his ability to drink water / eat.

Did you know that nerve entrapment of the superior labial branches of the infraorbital nerve can affect not only sensation but also the occupation of eating and facial expression? This nerve runs between the zygomaticus minor and levator anguli oris muscles, and compression can result in numbness or tingling around the upper lip, difficulty with chewing, and even changes in facial expression.

As occupational therapists, we understand the impact that nerve entrapment can have on daily activities like eating, drinking, and socializing. Our goal is to help individuals regain their independence and improve their quality of life. We can work with clients to develop strategies to modify their eating habits, such as using adaptive utensils or adjusting the texture of their food, and teach exercises to help restore facial muscle function and expression.

Remember, if you or someone you know is experiencing symptoms of nerve entrapment, seek the advice of a healthcare professional, and consider consulting with an occupational therapist to help manage the impact on daily activities.

Thanks to that I have possessed the knowledge and manual therapy skills to effectively treat soft-tissue/neurofascial/neuromuscular disorders in the whole body. It was a perfect combinations of refined learning of gross anatomy, palpation of dysfunction and pathology, and human kinesiology. It positively affected how I practice and got my patients better faster.

Next - keep getting better at the craft and advance in this skill.

Dequervain’s Tenosynovitis
Exotic sounding condition, commonly known as “texting thumb”, is caused by constriction/pinching of the wrist tendons at the thumb side. A sharp pain/discomfort can arise from such simple tasks as lifting a laundry basket, putting away groceries, writing, swinging a golf club or tennis racket, lifting babies, or what we do a lot nowadays, scrolling through instagram on our phone.

Occupational Therapists can detect the root causes of the pain and provide treatment to help you get back to doing things you love to do in life and avoid surgery.
A customized splint was made for this patient that immobilizes his thumb and part of the wrist to help control the pain associated with thumb movement. On top of some helpful stretches and manual massage, he was also advised to minimize certain activities that put stress on his wrist.

We look forward to seeing him making positive progress in his healing journey with us.

A thorough UE evaluation is key to successful treatment plan formation.

This pianist patient of mine came in with distal volar side wrist pain and thenar eminence intermittent spasm. After ruling out all the common causes, it was discovered that he has an anatomical variation that only found around 20% of the population.

➡️ Linburg–Comstock Variation/Anomaly

☝🏻Characterized by flexion of thumb causing flexion of the index finger with pain in the wrist and forearm due to a tendinous interconnection between the flexor pollicis longus and flexor digitorum profundus.

☝🏻In another word, one cannot isolate tip of thumb bending from tip of index finger bending, and vise versa.

☝🏻 Inflammation to tendons could develop after a forceful/repetitive extension the index finger with the thumb in a flexed position, which is common for a pianist.

Patient s/p basketball ball injury who developed SF pseudo boutonniere deformity with PIPJ flexion contracture due to delayed treatment. This is the 2nd session with me and after some good attempts, we finally able to fit him with a nighttime static progressive orthosis that is self-adjustable thru Neoprene strapping system. We look forward to seeing him with increased PIPJ extension weekly with this nighttime orthosis coupled with a daytime LMB splint.
