Our CISA team plays a vital role in observing and reporting changes, supporting home care workers, and connecting clients to the right services. Their work helps prevent hospitalizations, improve value-based outcomes, and strengthen the overall client experience. 💪

Our team completed a training from PHI, focused on stronger communication, accurate reporting, and early identification of client needs. Topics included:

• Active listening, effective communication & constructive feedback
• ORR/EORR and establishing client baselines
• Screening for Social Determinants of Health (SDOH)
• The Stop & Watch early warning tool
• Key health topics: Diabetes, Respiratory Disease, Cardiovascular Disease & Depression
• Real-life scenario practice
