The biggest driver in positive outcomes with persistent pain is not how strong you are.

It’s not your posture.

It’s not your alignment.

It’s not how you breathe.

It’s your self efficacy.
👉🏼 Or the belief in your ability to perform the necessary behaviors to produce a specific outcome.

The biggest gifts we offer people in pain is a listening ear, an attempt to make sense of their story, and a focus on all the things they can do.

It’s amazing to see how quickly perceptions can change.

What are “wins” in physical therapy?

The goal should rarely JUST be “being completely pain free”. If that’s the only marker of progress, I’d encourage you to dig a little deeper.

A post from a while back really stuck with me. He compared pain and rehab to financial investing - most people don’t get rich overnight. It’s the result of small gains compounding over time.

Rehab works the same way.

So what are the “compound interest” wins in PT?

- Getting stronger
- Tolerated movements more easily
- Recovering faster between exercise sessions
- Less frequent flare ups
- Less intense symptoms
- Feeling more confident moving again
- Less emotional response to the presence of pain
- Returning to activities or movement patterns you’ve been avoiding

The list goes on and on.

I’ve had this conversation many times with patients who feel they aren’t improving because some pain is still present - while they’re simultaneously doing things they couldn’t do weeks earlier.

Progress in rehab is often subtle before it becomes obvious.

Pay attention to the small wins, they tend to compound.

When pain feels dangerous, it gets louder.
When it feels manageable, it can start to quiet down

Pain isn’t just about alerting us of damage to tissues - it’s about perception, uncertainty, and threat.

Real reassurance is not:
❌“You’re fine, just push through it”
❌ Empty positivity
❌ “No pain, no gain”

Real reassurance is:
✅ Clear explanations
✅ Context around imaging and diagnoses
✅ A plan that builds confidence step by step

The goal is never just to reduce pain.
It’s to change how someone relates to it.

Sometimes knowing where to start isn’t the first step, it’s knowing that it’s okay to start.

Because when people feel safe to move again, everything changes.

❗️We have to do better!

First off, this isn’t to throw shade or

But I do think it’s a discussion we need to have more of:
1️⃣ Why do we make people think there is some special formula for pain?
2️⃣ Why do many clinicians blame them for not getting better?

❗️This isn’t a “physio” problem by the way, it’s a healthcare one.

We have all had this experience whether or not you’re a client or a therapist.

But why not ask:
1️⃣ Are the times we are scheduling the best for you?
2️⃣ Would virtual appointments be better for you to be consistent?
3️⃣ What opportunities do you have with work, childcare, etc?
4️⃣ How important is rehab for you right now?

🗣️ If you’re a clinician I want to know: What would you do in this situation?

🗣️ If you’re a client I want to know: Have you have ever felt blame as a client by a provider?

Everyone’s looking for the next pain relief hack…
♨️ Red light therapy.

🥳 From being afraid of walking around the block, to pushing her husband to go for an evening stroll every night!

🌎 Pain shrinks our worlds and ultimately becomes a big part of it.
👉🏼 But what if instead of trying to eliminate our pain in a small world we make our worlds so big that pain is only small fraction of it.

🫩 Too many clinicians are stuck fighting pain along with their clients that everyone ends up stuck, frustrated and unsure of what to do next.

✅ But when you realize that exercise, movement and simply living cannot only be a part of your rehab but also the pathway to your goals everything comes into alignment.

🗣️ Pain lies to us!

❌ It will make us think we can’t.
❌ It will make us think we shouldn’t.
❌ And then it will make us unable to do anything we love at all.

🚶🏻‍♂️ Walking is some of the easiest low hanging fruit clients are not taking advantage of when it comes to getting more movement and exercise in their life.
