Here’s an incredible case I wanted to share with you that Dr. Brian Choi has been handling.
Tooth #9 presented with extensive vertical bone loss extending almost to the apex. The prognosis looked hopeless at first glance.
Instead of jumping straight to extraction and implant, we took a conservative approach: conservative, non-surgical treatment, occlusal analysis with bite adjustment, and Invisalign to stabilize the occlusion and reduce traumatic forces. No surgery, grafting, or lasers were used.
The result? The tooth is now fully stable, periodontally healthy, with excellent bone support with a very favorable long-term prognosis.
This is exactly why early perio intervention and close collaboration with restorative dentists can change the trajectory of these cases. Sometimes the best implant is the natural tooth we manage to keep!

These lower front teeth were on their way out. A lot of dentists would have just pulled them and recommended implants.

Instead, a tissue graft was used to cover the exposed roots and build up the thin gum tissue. Two years later, the gums are thick, healthy, and holding strong.

I’m really happy with how stable everything looks. The teeth are still doing their job naturally, and we avoided jumping straight to implants.

Sometimes saving the teeth you were born with is still the best move, especially in the front lower area where bone and gum are limited.

Three months after a gum lift, the smile is even, more tooth is visible, and the proportions look harmonious without any restorations.
