I provided contractual services to this facility for survey prep- my engagement ended after the survey. My heart is with the family and loved ones affected by this tragic event. No one should ever have to receive a call like this. 🕊️
During survey prep, a lot was discovered-risks were identified, documented, and escalated-but, so to speak, many were left unaddressed once survey was over. This is exactly why risk management around accidents and incidents cannot be a “survey-season project”. When surveyors leave, resident care and safety must stay the priority every shift, on every unit, by every leader.
🚩Area of concern I continue to see across the board-starting with staff and leadership:
* Leadership accountability and real, lived safety culture
* Adequate staffing, skill mix, and active supervision
* Individualized risk assessments (falls, elopement, behavioral/cognitive) with timely care plan updates
* Behavioral health support, de-escalation, and 1:1 monitoring when indicated
* Roommate captibility, observation protocols, elopement prevent
* Incident/accident reporting, unbiased investigation, root-cause analysis, trend tracking
* Clear handoffs, escalation pathways, and on-call responsiveness
* Documentation accuracy. QAPI- follow-through, and learning loops after events
* Environmental safety rounding and prompt hazards remediation
