All hail the glorious granuloma, a type of inflammation that "walls off" an infectious organism. Granulomas (or more properly 'granulomata') are more commonly seen in mycobacterial (and some other bacterial) or fungal infections, but not really a feature of viral infections.These can be seen in non-infectious conditions as well (such as sarcoidosis).

Their most prominent characteristic is the presence of multinucleated giant cells, which can best be seen on the 3rd slide.

This granuloma is present in the lung, an extremely common place to find them; in Indiana (where there are environmental organisms such as histoplasmosis), it's exceedingly common to find them and it really doesn't have much clinical significance. I'll post my chest xray sometime and you can marvel at the number of granulomas.

A 16-year-old girl presented with a 2-month history of cough. Chest radiography showed a well-circumscribed cystic lesion containing another rounded structure, creating a double-domed appearance, at the base of the left lung.
