This patient had an to address multiple parts of his nose, but was mainly bothered by the droopy nasal tip. Nasal tip support is extremely important to maintain the harmony and longevity of the nose.

Immediate after photo of a closed rhinoplasty performed in under two hours. ⏱This is sometimes called a scarless rhinoplasty as there are only incisions hidden internally. I like this technique for patients whose main concern is the dorsal hump and who want to avoid overly manipulating the nasal tip. I pair this with some tip elevation and support to balance the nose make sure it doesn't look too long and swooped. If you made it reading this far, you might as well call.

Thank you and for including me in this piece. Non-invasive procedures certainly have their place in early treatment or as an adjuvant to surgery. They are unfortunately not magic and cannot do everything for everyone. A proper surgery will often do more in less time and still look natural. As a surgeon, I can help guide patients to that decision rather than continue down a seemingly endless road of "new tech" promises.

We had a very special visitor to the office, my fellowship director and mentor . Coming all the way from in Beverly Hills where I trained. We discussed all things rhinoplasty, preservation rhinoplasty, and even "preservation" facelifts-- which is a variation of the extended deep plane composite lift developed by Frank Kamer at Lasky Clinic decades ago. There is no East Coast - West Coast conflict when it comes to surgery.

An unassuming, less dominant nose releases the rest of the face for interaction. ❤️👃🏻

result reducing the convexity of the nose and stabilizing the tip so it doesn't dip down when smiling. 🙂

Increased nasal support also prevents a droopy tip as we age -- it's no such much that the nose grows with age, but rather loses support and elongates.

📸 Honest photography is a big issue when choosing a doctor. Everyday I have people share pics of "amazing" results from around the world. A lot are great--many are deceptive (and that's being polite). Here's how you can tell:

Proper Photos:

✅ Same camera and lens
✅ Proper lens size to avoid distortion
✅ Lighting avoiding shadows
✅ Same background

Questionable:

🚩 Different lens for before and after- warps faces makes it look worse in the before.
🚩Lighting - Different or improved lighting on after to make it better- can see in eye reflex
🚩Proper before and candid after-- Everyone can find one angle that looks great regardless of the rest.

I spend a lot of time making sure our photography is honest and consistent. It is a little scary to see questionable photos out in the world that can confuse or entice patients.

You are what you do, not what you say you'll do. Understanding patients' goals is only the first step. You can see how close we came to the simulation in this post. Not every patient wants the same nose and we respect that. Every rhinoplasty has specific challenges that can limit the final result. Taking the time to explain realistic outcomes is key.

Surgical results on a happy patient. Went with an open functional approach which also restored breathing. I haven't labeled which is the before and after but hope you will know. It's unclear if he had to reset his FaceID.

This lovely patient is about three weeks post deep plane face and neck lift surgery.
