IS THERE REALLY A “SMAS”?

Nearly 50 years after Dr. Mitz introduced the term, surgeons still debate the anatomy, the planes, and even the nomenclature itself.

Is it a distinct layer?
A transition zone?
Different in the cheek than the neck?

The truth is, anatomy is far more dynamic than any textbook diagram.

The deeper we dissect, operate, and study the face, the more we realize facial rejuvenation is not about a single layer — it is about continuity, retaining ligaments, vectors, fat compartments, fascia, movement, and artistry.

In the end…

it’s the singer, not the song.

The last three and a half weeks have been an incredible reminder of why I love this field so deeply.

From our own NYC Facelift & Rhinoplasty Master Course in New York City… to Deep Plane Brazil… to the Cairo Face Meeting, where I lectured on advanced deep plane facelifting, defining the ideal neck, endoscopic brow lifting, preservation brow lift techniques, and endoscopic facial rejuvenation… and then to the American Society for Aesthetic Plastic Surgery meeting in Boston this past weekend, sitting on a distinguished panel among leaders in aesthetic surgery…

…each meeting brought together thousands of surgeons from around the world, all united by one goal: advancing our specialty and improving outcomes for patients.

Teaching has become one of the greatest passions of my career. Every lecture, panel, cadaver lab, and discussion pushes all of us to think deeper, refine techniques further, and continue innovating.

As Rumi said:

“A candle never loses any of its light while lighting up another candle.”
