How to Make IVF Affordable with DOR

Diminished Ovarian Reserve (DOR) presents one of the most challenging scenarios in fertility care: not only because of the biological limitations but also due to the financial burden it places on patients. Understanding how to make IVF affordable and effective with DOR requires both medical expertise and individualized strategy.

While conventional thinking may suggest increasing FSH to push the ovaries into producing more follicles, Dr. Luk points out that many ovaries with diminished reserve are already maxed out, regardless of medication dosage. In some cases, using high-dose FSH not only fails to increase follicle count but may actually result in a suppression effect, where the ovaries respond even less favorably. It’s not about forcing more; it’s about optimizing what the body can naturally offer.

This perspective not only respects the biology of DOR patients but also makes IVF far more financially sustainable. Patients can avoid unnecessary spending on aggressive medication protocols that may yield no additional benefit. Instead, a smart, evidence-based plan tailored to the individual can offer both emotional and economic relief while preserving the best chance of success.

One of the most common questions among patients exploring fertility treatments is: Why is IVF so expensive?

The answer lies in the complexity of the treatment process: particularly the cost of fertility medications, which make up a significant portion of the overall expense. These medications are essential for stimulating the ovaries to produce multiple eggs: a key component of a successful IVF cycle. Depending on a patient’s individual protocol and response to treatment, several pens may be required, quickly escalating costs.

Understanding the breakdown of these expenses can help patients prepare financially and explore potential strategies to manage the burden (such as generic medication options, pharmacy discounts, or insurance reimbursement where available).

IVF Medication Tips

Want to get the most out of your IVF meds without overdoing it? 💉💊
✅ Don’t overstimulate: your meds should match your ovaries' response
✅ Adjust protocols as you go, not just at the start
✅ Use oral meds (like Letrozole) when possible to cut injection costs 💸

It’s all about working with your body, not against it 🙌

Who is a Good Candidate for Low-Cost IVF?

IVF isn’t one-size-fits-all, and for many patients, lower-cost alternatives like Mild IVF or Luteal Phase Stimulation can offer a more personalized, affordable, and effective treatment option. These protocols are often gentler, use fewer medications, and are particularly well-suited for patients who don’t respond well to conventional high-dose stimulation.

1️⃣ Patients who have failed conventional stimulation protocols. If high-dose IVF hasn’t worked, trying a mild or Luteal Phase approach may provide a new path forward—one that is better aligned with your body’s natural rhythms.

2️⃣ Patients with Diminished Ovarian Reserve (DOR). A gentler approach may yield better quality eggs and avoid the physical toll and financial burden of high-dose cycles that don't improve outcomes for this population.

3️⃣ Patients who feel unwell or overstimulated on high-dose medication. A low-stim or natural protocol can be both safer and more comfortable—without sacrificing results.

"Can I still get pregnant with DOR?"

A diagnosis of Diminished Ovarian Reserve (DOR) can be emotionally challenging, but it does not mean the door to pregnancy is closed. It simply means that your ovaries are producing fewer eggs than average, which may affect the number of eggs retrieved during a fertility cycle but doesn’t necessarily impact your ability to conceive.

DOR refers to egg quantity, not egg quality... and pregnancy is still possible with fewer eggs, especially when guided by an individualized treatment plan.

While those with DOR may require different stimulation protocols or more cycles to achieve the desired outcome, many go on to have successful pregnancies. Understanding the condition, staying proactive, and working with a fertility team that tailors care to your specific reproductive profile can make all the difference in your journey.

Ways to Make IVF Affordable with PCOS

One key strategy for making IVF more affordable and more effective involves customizing the medication protocol to reduce unnecessary drug use without compromising outcomes. Certain PCOS patients benefit from a lower-dose stimulation protocol that incorporates Letrozole, an oral medication typically used to induce ovulation. Unlike protocols that rely solely on injectable FSH (follicle-stimulating hormone), adding Letrozole can help stimulate the ovaries more gently, reducing the risk of overstimulation.

This approach isn’t just biologically sound, it’s also cost-effective. By reducing the total amount of injectable medication needed, patients often see significant savings on drug costs. And beyond saving money, this modified protocol can sometimes lead to better egg quality, which is especially important for improving IVF success rates and reducing the risk of complications like ovarian hyperstimulation syndrome (OHSS).

For patients with PCOS, a personalized treatment plan that accounts for their hormonal profile, ovarian reserve, and financial considerations can make all the difference in their fertility journey.

41, Single & Unsure?
Here’s What Dr. Luk Recommends

For women in their early 40s who are single and haven’t yet frozen their eggs, fertility decisions can feel overwhelming. The biological clock becomes more pressing, and uncertainty about future relationships often adds an emotional layer of complexity. Still, options do exist, but timing is critical.

Dr. Luk encouraged viewers to reflect seriously on whether having a child is something they truly want. If the answer is yes, she said, it’s worth considering becoming a “single mom by choice”: a path many of her patients have taken and found deeply fulfilling. “They’re very happy,” she noted. “Obviously they would love a companion, but sometimes when you don’t have a partner and it’s something you want, you never want to lose that chance.” She emphasized the importance of a strong support system from family and friends to make solo parenting feasible.

For anyone in this situation, it’s important to consult with a fertility specialist right away. Fertility declines sharply after age 40, and delaying decisions can limit your options. Egg freezing, embryo creation, or even moving forward with donor s***m are all possibilities, but they require timely action.
