Lingual (tongue) cupping is a reflex in the infants mouth which triggers the tongue shape into a bowl position. The tongue lifts up and cups forward on to the ridge of the gum line. This is one of four markers we assess in tight latch, suck and seal during infant feeding.

Typically we look for four different inter oral movement markers when assessing a latch or suction.
1. Lingual cupping - described above
2. Lingual lifting - the upward movement of the tongue to the top of the mouth
3. Lingual Extension - the exertion of the tongue to the gum line
4. The rhythmical wave - the movements during the coordination of oral markers 1-3.

When babies come in to our practice we assess the movements and coordination of these markers to determine causations and correlations for feeding and latching challenges. Then we work on therapeutic techniques to help create good latch and refer out when needed.

Invariably, we also ask questions about prior sucking and latching history for children coming in with feeding issues at ages 6 months and up. By looking at the history of feeding challenges we can narrow down anatomical issues from physiological issues which might be contributing to the challenges during mealtime. When families come to us looking for an apraxia evaluation we can help BUT if the child is thumb sucking or is on the pacifier it is extremely difficult to parse out the resulting errors from apraxia and the errors resulting from a muscular disorder like tongue thrust and learned low resting posture of the tongue.

This two year child came to us in January with and potential diagnosis of apraxia which we later diagnosed in February.

However it was difficult to determine where errors resulted from. Now that the child has been free from the pacifier for a few months we are working on a combination of apraxia treatment and orofacial myology treatment.

Hey, I am Rebecca Taskin I’m a licensed pediatric speech and therapist feeding therapist in New York City. Our clinic os growing and we are currently a team of 10 women working to help kids develop their speech and feeding abilities.

I am certified in PROMPT therapy as a motor planning intervention for children with speech-sound disorders, and have worked with children with a wide variety of diagnoses, including Childhood Apraxia of Speech, Angelman’s Syndrome, Cerebral Palsy, Autism Spectrum Disorders, Down Syndrome, Pallister-Killian Syndrome, Hydrocephalus, Infantile Strokes and Seizures, Dandy-Walker Syndrome, Sturge-Weber Syndrome, and various genetic disorders.

I am a trained feeding therapist and work with babies to young adults who have different types of swallowing disorders, from sensory impairments to motor disorders. I asses, treat, and manage different types of dysphagia, collaborating with nutritionists, physicians, and parents to work towards meaningful outcomes. I have been the lead therapist on several g-tube and ng-tube weans and has been is trained in several methods for sensory feeding therapy and responsive feeding therapy.

I am certified in Fiberoptic Endoscopic Evaluation of Swallowing (FEES) treatment. I am trained in the treatment of Orofacial Myofunctional Disorders for speech and swallowing by the IAOM (tongue tie, lip tie, buccal tie, thumb sucking, tongue thrust, etc.)

I specialize in AAC (Augmentative and Alternative Communication) and am a licensed ATP (additive technology professional). I have extensive experience with PRC and Tobii Dynavox Products, as well as various related software. I evaluate, asses, and treat children and young adults who use assistive technology, including evaluations and trials for switch site and device access, and treatment plans for use with high-and-low-tech speech generating devices (SGD). I have developed and implemented treatment with tangible symbols for children with visual impairments including blindness and CVI (Cortical Visual Impairment)

I’m really excited about this…We are so excited to be one of a very few practices offering therapy in a group setting for picky eaters. These classes are run by a speech pathologist and an occupational therapist.

This class focuses on exposure to foods, setting the stage for picky eaters to explore their senses. Children explore and get the opportunity to get messy with food. This low pressured environment helps support our brave little ones interests and provides many interactions with foods of all types. This class is open to all, especially picky eaters!

FAQS
Where are the sessions held? All sessions are held in our office 30 East 60th Street, Suite 904

How long is the session? All sessions are 45 minutes.

How much is the session? All sessions are $50 each.
You will receive an invoice after the class.

How many children are in a class? We are only accepting a maximum of 8 kids at a time during the class.

Can I stay during the session? Not at this time, we are asking parents to wait downstairs as our office is small and we are taking COVID precautions as needed.

Do I need to bring anything with me? No, all material/foods will be provided.

Do I need to sign my child up for the whole semester? No, but you are welcome to!

How will this help me feed my child at home? We send home pamphlets with information on the session, what you can do to carry over the session and will set up phone calls with you as needed.

What is your cancellation policy? If you have to cancel, we ask that you do within 48 hours as spots are very limited.

This class is open to all.

A one year old will most likely not blow out their birthday candles, but by two they should be able to according to developmental norms in the U.S.

If a child has limited speech and/or poor speech intelligibility (no matter what age) one of the first things I asses is, if they can sustain air flow… AKA their ability to blow A controlled air stream is one of the first steps to verbal speech. If not present yet, do lot of activities in our sessions to help support this skill such as, practicing breathing, whistles, blowing bubbles, and manual compression techniques.

This can get missed in older kids who are speaking a lot, but may not be clear. Having a clear and sustained “H” “A” and “M” can be a good clue into what’s going on internally.

Managing child led play during treatment of childhood apraxia of speech can be tricky but not impossible.

A majority of therapy sessions are driven by repetition and rehearsal for treatment of childhood apraxia of speech as well as development of the child’s play skills. That means I’m trying to let them be a child while at the same time getting in many drills.

The activities I bring or use, require maximum repetitions and opportunities for turn taking. This means while I let the child lead HOW we use the toys and WHAT they want to do with them… I lead HOW we say the target words and WHAT the target words are based on what I think is appropriate for that child. This word list is based on observation and assessment of the child’s speech system in connection with the activity.

The targeted words should be specific to the activity have have relevancy during the time so that the child can use them right then in play…. This is why we don’t use random words and instead provide words that are in context for their here and now.

Take turns during play. It sounds so simple, but during turn taking, we are investing in their interests and creating circles of communication. Even your child isn’t using verbal speech yet, taking interest in their play is building on their narrative. Stars if:

⭐️ You are narrating the whole play scheme, “Your turn, oh you’re putting the yellow block on top, My turn to put the red block on top”

⭐️ You sit at eye level with your child, so they are able to see your mouth move

⭐️ You label/ point and pause before you take your turn them “should I pick red or blue… hmmm”

⭐️You over emphasize silly things and mess ups. Change your intonation during play. “Oops too big”

⭐️ You let them figure out how things work and fail. Let them build the tower so high it falls down.
