There’s a lot of confusion and anxiety around prostate screening, so when I was asked to contribute to this Men’s Health article, I welcomed the opportunity to address some of the most common misconceptions.

Much of the concern comes from not understanding what the blood work measures, why it’s ordered, or how results should be interpreted. This test looks at a protein produced by the prostate to help assess risk, not to give a yes or no diagnosis. Context matters, including age, symptoms, family history, and how values change over time.

Screening today is far more individualized and evidence based than many men realize. When done thoughtfully, it helps identify who needs closer evaluation and who can be safely monitored.

If you’ve been putting off screening or aren’t sure what’s right for you, this is exactly the type of conversation we have in my practice. Education comes first, decisions come second, and your personal risk always guides the plan.

Erections are one of the most sensitive vascular functions in the body. That’s why diet shows up here first.

In a large JAMA Network Open study, men with higher adherence to a Mediterranean-style diet had a significantly lower risk of erectile dysfunction, even after adjusting for age, activity, and smoking. The biology tracks: endothelial dysfunction, impaired nitric oxide signaling, insulin resistance, and inflammation all reduce pe**le blood flow long before they cause overt heart disease.

As a urologist, this matters because erectile dysfunction is often an early warning sign, not an isolated complaint. Pills can improve symptoms, but lifestyle and vascular health determine trajectory.

If erections have changed, it’s worth asking why. A proper evaluation should look beyond testosterone and prescriptions to cardiometabolic risk factors that can be modified early.

Aging influences hormones, but function tells the full story. In clinical practice, the focus is not age alone, it’s how well the body’s systems are working together over time.

Testosterone reflects the health of multiple systems, including:
• Sleep and circadian regulation
• Metabolic and insulin signaling
• Body composition and muscle mass
• Inflammatory load
• Cardiovascular and vascular health

When these systems are supported, men often maintain strength, energy, cognition, and s*xual function well into later decades. When they’re disrupted, symptoms appear long before testosterone falls outside a lab reference range.

If something feels off, don’t assume it’s just age. A thoughtful evaluation can clarify what’s normal, what’s modifiable, and what deserves treatment.

Declining estrogen affects the bladder, urethra, pelvic floor, vaginal tissue, sleep regulation, bone density, and cardiovascular risk, which is why symptoms extend far beyond hot flashes.

When fewer than 30 percent of women receive evidence based treatment, the result is delayed care and progression of symptoms that often become more complex over time. From a urologic perspective, untreated menopause commonly presents later as chronic dryness, painful in*******se, recurrent urinary symptoms, urgency, incontinence, and tissue fragility that could have been prevented or significantly improved earlier.

Education matters because early, appropriate treatment can preserve tissue health, improve quality of life, and reduce long term complications. Menopause is a medical transition with well studied therapies, not something women should be expected to manage without guidance or care.

Most women think about calcium when it comes to midlife health but protein is just as important 🥩

During perimenopause and menopause, protein doesn’t just feed muscles. It also supports bone strength, hormone balance, immune function, and recovery from daily stress. Without enough, women are more likely to experience fatigue, cravings, slower healing, and even greater bone loss 🦴

The easiest fix? Build your meals around protein first. Add eggs to breakfast, Greek yogurt as a snack, salmon or beans at lunch, and chicken, tofu, or lentils at dinner. Little shifts like this can have a big impact on strength, energy, and resilience over time.

The result? Decades of gaps in understanding, limited treatment options, and a lack of evidence-based guidance for millions of women.

Women are too often left to “figure it out,” when what they deserve is rigorous science, thoughtful care, and physicians who take their concerns seriously.

The bottom line: menopause is not a niche topic. It affects half the population, influences some of the most costly age-related diseases, and directly shapes women’s quality of life for decades. It’s time funding, research, and medicine reflect that reality.
