The Caregiver Program of SNAP

The Caregiver Program of SNAP Providing support for informal caregivers with services at home, in the community, and by phone. We serve Queens Community Districts 8–14.

Respite • Supplemental Services • Individual Counseling • Support Groups • Educational Workshops • Case Assistance Hours of operation have changed due to Covid-19. The office is open from 8:30am to 4:30pm Monday, Wednesday and Friday. We service Community Boards 8, 9, 10, 11, 12, 13 and 14. To join the program call us at 718-527-5380. We will do a simple intake over the phone and then a case worker will come out to meet you and your love one in the home. Due to Covid-19 assessments can be done over the phone. At that point you can take advantage of any or all of the services listed. We look forward to working with you and your loved one.

To our participants, caregivers, families, volunteers, and supporters—thank you for being the heart of SNAP. Thank you for the trust you place in us, the moments you share with us, and the community we continue to build together.

We see love every day in the way people show up for one another: in patience, in compassion, in laughter, and in care.

Yesterday, we showed up. 💙

Alongside the Age Strong New York Coalition, we’re advocating for something simple:
Older adults and caregivers deserve real support — not just a plan on paper.

New York has already created a Master Plan for Aging, but without funding, it’s a roadmap without fuel.

We’re calling on the Legislature to fully fund Proposal #50, investing $173.5 million to strengthen community-based aging services — the programs that keep older adults healthy, independent, and connected at home.
Because aging in place shouldn’t be a privilege.
Because caregivers shouldn’t have to do it alone.
Because prevention and community care are smarter, more cost-effective, and long overdue.

When we invest in meals, home care, transportation, caregiver support, and local services — everyone benefits.

We’re proud to stand with advocates across New York to make our voices heard. 🗣️

It’s Therapeutic Recreation Month!

Therapeutic recreation is about so much more than activities. It’s about creating opportunities for connection, purpose, movement, creativity, and joy — and supporting the whole person: mind, body, and heart.

At SNAP, therapeutic recreation is woven into everything we do. From art and music to movement, games, and social groups, these moments help reduce isolation, support cognitive and emotional well-being, and remind people that they are seen, valued, and part of a community.

Mental Wellness Month
Week 1:

Pause + Breathe.

Caregiving can feel nonstop, but even a few intentional breaths can help calm the body and mind.

Try one of these gentle breathing techniques whenever you need a moment:

🌬️ Box Breathing
Inhale 4 • Hold 4 • Exhale 4 • Hold 4

🌬️ 4–6 Breathing
Inhale 4 • Exhale 6
(Longer exhales help signal calm)

🌬️ Hand-on-Heart Breathing
Place a hand on your chest.
Breathe slowly and notice the rise and fall.

🌬️ Counted Breaths
Inhale 1… Exhale 2…
Continue up to 10, then start again.

You don’t need to fix everything — just breathe.
