This is a patient of Dr Jason Hoellwarth. This is a good looking xray, the legs are equal length with straight weight bearing line from the hip through the center of the knee to the ankle. But this teenager started with both legs bowed way out here with a leg length difference. He walked with pain and struggled to get through each day. Thankfully, Refuah Helpline connected him with me. I counseled him that I could help him, one leg at a time, by reshaping his bone as shown here. On the left side I would remove a wedge of bone from above his knee and stabilize him with a plate. One his right side, I described more was needed. I would remove the bone from the femur to straighten it, and insert it into the tibia make it straight as well and also hasten his healing. I performed his two surgeries, and now his legs are straight, leg lengths equal, he is walking great, long distances without pain, and it’s time to remove the plates!

One of Dr Jason Hoellwarth's patients. That is how Mr J feels today. But the gait you’re watching is how he was when he presented. He swung his left leg to the side and had bad stiffness and pain. An xray revealed he had a large bony mass in his ankle which developed after his surgeries. I performed a short surgery to remove that bone. You can see in this postoperative xray that the bone is now gone. And he walks so much more comfortably today!

Dr Hoellwarth's video shows a common injury in an uncommon patient for that injury. Mr B is a young adult who fell off a hoverboard and had a painful right hip. He was initially seen elsewhere and informed it did not appear fractured, but continued pain while walking on it for several days made him seek out another opinion with me. His hip CT further confirmed the fracture. I took him the next day for surgical fixation with this minimally invasive small plate and screw. Here is a series of intra-operative images showing holding the fracture in place followed by preparing the fixation path and then applying the bolt and plate and screws. I permitted him progressive weight bearing after surgery and now two months later he is walking normally

This is Mr N, one of Dr Hoellwarth's patients. He is Amish and was happy to share his experience with his face protected. He had a childhood wagon injury with multiple infections and short femur #amputation and painful #heterotopicossification which you see circled in red and that I’m pointing at, which made socket prostheses wear difficult, very painful, leading to very low mobility. When he did walk, he had a bad limp due to the socket sliding around. He wanted #osseointegration. Here is a clip of the surgery where I removed a huge piece of painful bone. You can see it’s not there on his post-op xray. Soon after the surgery his pain had improved significantly. Here he is at the office of his prosthetist Kent Turnbow in Indiana a few months later when he was fit with a prosthetic leg and began re-learning to walk. Now at almost 18 months, he wrote me this wonderful message to update me.

Dr Hoellwarth treated Mr A, who is an older man who had a right transfemoral #amputation #amputee due to #limbtrauma over 40 years ago. He had been making do with a socket prosthesis, but he was especially frustrated by how long it took to put on. He asked me if #osseointegration ntegration was ok for someone who had been an amputee as long as he has been. I said of course it is! Here he is just six weeks after surgery, starting the loading protocol. A quick six weeks again and he was standing with level hips and putting the leg on for the first time! He’s now four months after surgery and it is hard to tell which leg has the amputation! More amazing, after this entire story, his old self still hasn’t even finished putting his socket on!
