💡 CPAP vs MAD: New Evidence in Severe Sleep Apnoea
Severe obstructive sleep apnoea (OSA) isn’t just about tiredness — it’s linked to high blood pressure, heart strain, and long-term cardiovascular risk.
Until now, CPAP (Continuous Positive Airway Pressure) was considered the gold standard.

But new research shows that Mandibular Advancement Devices (MADs) — small oral appliances worn during sleep — can be a powerful alternative.

🧠 The Study
A randomized 12-month trial followed 144 patients with severe OSA, comparing MADs to CPAP.

Both treatments improved sleep quality — but there were surprising findings:

📉 Blood Pressure:
MAD therapy significantly reduced night-time blood pressure, while CPAP showed no significant change.
Asleep mean BP ↓ 4.7 mm Hg
Asleep systolic BP ↓ 2 mm Hg
Asleep diastolic BP ↓ 4 mm Hg

😴 Sleepiness & Quality of Life:
Both groups felt better. CPAP improved daytime sleepiness slightly more, but overall quality-of-life gains were similar.

❤️ Heart & Safety:
No major heart rhythm or imaging changes.
MAD users reported mild jaw discomfort (~15%), CPAP users more dryness, congestion, and air leaks.
