• Understand HMO policies, managed care processes, and case management guidelines
• Collaborate with care teams, providers, and support departments
• Maintain high standards in documentation and communication
• Use Microsoft Office and other tools for reporting and documentation
• Recommend improvements to the case management program
• Prepare and organize monthly, quarterly, and annual reporting logs
• Participate in Interdisciplinary Care Team meetings and prepare case summaries
• Coordinate special projects assigned by case management leadership
• Track and report on cases for assigned groups or networks
• Communicate contracting-related updates and follow-ups
• Support committees and initiatives that enhance patient care
• Identify community and family support resources to improve continuity of care
• Coordinate with hospitals, physicians, and facilities regarding referrals and care transitions
• Provide accurate information to patients about benefits, resources, and referrals
• Assist in coordinating services for high-cost or high-utilization cases
• Ensure compliance with all state, federal, and health plan requirements
• Assist in preparing for audits when needed
• Support onboarding and training of new team members
• Perform additional duties as assigned
