So, I keep getting asked about this "NEW" medication called Tonmya coming out in late 2025 or early 2026 for fibromyalgia. So here is some brief preliminary information and thoughts.

AI Overview: Tonmya's active ingredient is cyclobenzaprine, a tricyclic compound that acts as an antagonist at several neurotransmitter receptors. While the precise mechanism for treating fibromyalgia is unknown, it is thought to improve sleep quality by modulating these receptors, which in turn helps reduce pain.

Receptor antagonism:

Through in vitro pharmacology studies, cyclobenzaprine has demonstrated functional antagonism at four key post-synaptic neuroreceptor subtypes:

Serotonin-5-HT2A: Cyclobenzaprine blocks this receptor. Since activating it has been linked to increased pain sensitivity in fibromyalgia, blocking it is believed to help relieve pain.

Alpha-1 adrenergic (α1): Blocking these receptors can lead to sedation, which helps improve sleep disturbances commonly associated with fibromyalgia.

My Initial Thoughts: Tonmya is just a sublingual (under the tongue) formulation of cyclobenzaprine, which is a different delivery from the traditional oral form (Flexeril). This is an old muscle relaxer medication being re-used, re-purposed, and re-approved for another indication (FMS). It is far from a new medication. This is very disappointing to me. Flexeril was tried for many years in FMS without many good benefits, unless the person also had significant muscle spasm. Remember, fibromyalgia is NOT muscle spasm. It never got to the root of the issue. It did, however, make people sleepy. It seems that they are trying this form to try and help promote better and more restorative sleep and hope that helps the FMS patient with their symptoms. It may help some in this way, but this is far from a new or novel medication getting directly to a newly understood root-cause reason for the disorder with a novel mechanisms of action. have my significant doubts that this will move the dial for most FMS patients, but it will breathe new life and revenue into an old medication for sure.
