In 2014, Staten Island Performing Provider System (SI PPS) formed an integrated network of medical, behavioral, and social services agencies under the New York State Department of Health Delivery System Reform Incentive Payment Program (DSRIP). Our founding goals included improving the quality of care, reducing costs and improving health outcomes for Staten Island’s Medicaid and uninsured populations. By the end of the DSRIP Program, we exceeded our goals and maintained our network to continue enhancing and refining the transformative work built in the Staten Island Community. Today, we continue to work with our partners to improve population health outcomes, address social determinants of health, grow our network and reduce health disparities. We are dedicated to improving health equity by holding conversations with our community, creating educational and workforce opportunities for youth, and bridging connections with non-traditional service providers to meet people where they are in the community.

If you have Medicaid, you might be eligible to use free transportation services through the Social Care Network. Qualifying members may be able to get ride assistance to appointments such as:

Housing appointments
Local Department of Social Services
Childcare / Parenting classes
Education and support for chronic conditions
Food pharmacies, farmer's markets, mobile markets, etc.

SI PPS and H.E.A.L.T.H. for Youths is hosting a winter coat drive on Tuesday January 27th! We'll have additional winter accessories available and will be providing screenings for the Social Care Network. If you have Medicaid, the Social Care Network may be able to provide you with FREE services including food, housing, and more.

We will also have staff on site to help you enroll in Medicaid if you need assistance.

With 2025 coming to a close, the Social Care Network has completed its first year, and what a great success it’s been! From all of us at SI PPS, THANK YOU for your continued support in this program and making it possible for us to give back to our community in all the ways they deserve. The Social Care Network has been a great testament to the importance of health-related social needs, and will continue to improve lives as we work together. Here’s to helping even more Medicaid Members in the coming years and showing up for one another!🎆🥂
