US Family Health Plan Uniformed Services Family Health Plan is a TRICARE Prime option for active duty family member retire

US Family Health Plan is a Department of Defense Designated Provider of the TRICARE Prime Health Program and has been providing care to the military community for over 30 years. Health care is offered to active duty family members, retirees and their family members, including active and retired National Guard and Reservist who reside in New Jersey, NYC, Westchester, Orange, Rockland, Suffolk and Nassau counties, Eastern Pennsylvania and Western Connecticut. The benefit design is that of an HMO. 100% of the doctors with USFHP are civilian. Members select and are assigned to a primary care physician (PCP) in their local area to provide and coordinate medical care. Covered benefits include outpatient and inpatient services, preventive care, skilled nursing and home health care, hospitalization coverage, maternity and infant care, behavioral health care, diagnostic tests and services, emergent and out of area care, physical, occupational and speech therapy, durable medical equipment also annual eye exams and now $0 to low cost eyeglasses!
