Partners Health Plan is a New York based non-profit organization that is proudly offering the first managed care program designed exclusively for Medicare and Medicaid eligible individuals with intellectual and developmental disabilities in NYC, Long Island, Rockland, and Westchester. PHP Care Complete (Medicare-Medicaid Plan), Your Plan, Your Way! PHP Care Complete FIDA-IDD Plan offers all medical, dental, behavioral health, Part D, and long-term supports and services covered by Medicare and Medicaid, including OPWDD services. The actual benefits and services that a participant receives is based on his or her assessed needs and personal preferences. Each participant will have a Clinical Team Leader and Care Manager assisting in the development of a customized Life Plan that meets your personal goals and valued outcomes. To enroll, call NY Medicaid Choice at 1-844-343-2433 (TTY: 1-888-329-1541), Monday through Friday, 8:30am to 8pm, and Saturday, 10am to 6 pm. This is not a complete list. Effective December 31, 2025, the PHP FIDA-IDD demonstration plan is no longer available. Earlier this year, all members received a CMS Non-Renewal letter with details about this change.

Because your current plan is ending, you have a special enrollment period that allows you to choose a new Medicare health and prescription drug plan anytime through February 28, 2026.

If you have not selected a new plan, please contact your Care Manager. They can help you review the CMS letter, understand your options, and support you through each step of the transition.

Your Medicaid coverage is not changing. You will continue to receive OPWDD services and supports through New York State Medicaid.

As the year comes to an end, we’d like to thank every member and family who has shared this journey with us to make the work we’ve done possible. We’ve achieved outcomes that truly reflect the power of person-centered care and built connections with members, families, providers and legislators that will continue to shape the future of care for people with I/DD across New York State.

While this chapter of PHP is coming to a close, our mission is not. Our team at Care Design New York will continue to champion and advocate for the I/DD community. Our commitment to integrated, whole-person care — and to the people we serve — will continue to guide everything we do.

As we reflect on 2025, we’re grateful for the many moments that brought connection, joy, and purpose to our community. From joining Pride celebrations, supporting cost-of-living adjustments and advocating for increased funding, to writing legislators and standing up for the rights of people with I/DD, your engagement made a real impact this year. And alongside it all, the excitement of the New York State Special Olympics Fall Games reminded us of the strength and spirit within our community. ✨

Did you know people with I/DD are at a higher risk of serious health problems from the flu? This week is National Influenza Vaccination Week, and with the holiday season upon us, now is one of the most important times to get vaccinated.
