Removal of hypertrophied bone great toe.. cheilectomy, this patient had severe degenerative changes, with severe pain greater than a year. Little or no motion was present at the great toe. Status post trauma, fracture present of the great toe( hallux) While working a 350 lbs of metal, broke thru a box landing on his great toe, severe bleeding under the nail (hematoma) and a longitudinal fracture of proximal phalange with another fracture fragment. Positive crepitus is present, immobilized the great toe, MRI was ordered to evaluate the fracture and assessing the damage to the articular cartilage. There is a possibility a fusion surgery ( arthrodesis) may be needed if the patient continues with pain. DrTotten.com

Complete rupture of Achilles tendon at the insertion with the calcaneous (heel), status six months still complaining of decrease strength and pain . Ambulated with a severe limp. This patient has a classic dell of the skin of the tendon An .. abnormal increase position in the dorsiflexion of the foot to the leg. . Instability, unable to run or stand in his toes, walks with a severe limp. Diagnosis is rupture of Achilles tendon at distal insertion. This patient has severe pain in the second interspace. (Yellow arrow) This patient has a classic “molder’s click” of the digital nerve. By squeezing the foot you can feel the classic click of the nerve rolling against the metatarsal head. There is a painful impingement of this plantar digital nerve between the two metatarsal heads. Treatment includes , anti inflammatory medication, steroid injections, orthotics and possible surgical excision of this painful plantar nerve.
