Patient an obese 52 y/0female stepping off of a curb, twisted her foot in an inverted position. She heard a crack! Patient has a high arch foot type (peas cavus) thereby increasing this forceful inverted force as she came off the curb. Patient was treated with soft cast to decrease the swelling, ice applied soft cast was for the swelling and cast boot will be needed to decrease the abnormal force of the peroneal brevis tendon. Painful sublingual exostosis is a hypertrophy of bone at the tip of the toe. It happens under the nail hence the term “subungual” it’s a benign tumor from the bone and cartilage. The cause is unknown, possible causation could be trauma, subungual exostosis may occur at any age and woman are more commonly affected. A firm nodule develops under the nail causing severe pain with separation of the nail from the nail bed. (Onycholysis) X -rays confirm the diagnosis.
