Dear Patients, Friends, and Colleagues,
It is with mixed emotions that I am announcing my retirement and closure of Newburyport Family Practice, effective April 25, 2025. It has truly been an honor and privilege providing for your health care needs over the years.
Selecting a new primary care physician is a very personal choice and ensuring you continue to receive the highest quality care is of the utmost importance to me. My goal is to work with you to transition your care to a new primary care provider who meets your needs.
You have choices about your health care, so I have included a list of primary care clinicians in the community that are accepting new patients:

If you have an insurance plan that requires you to select a Primary Care Physician, you will need to contact the insurance company directly to inform them of your new primary care provider selection. Please do not delay selecting your new P*P and take note of these important points that relate specifically to P*P selection:
● Please plan in advance for any prescription requests that you will need. Please contact my office to request your prescription refill prior to April 25, 2025. You will need to set up an appointment with your new P*P prior to requesting any refills.
● Referral authorizations are specific to a P*P so as of April 25, 2025, if you have a specialist appointment that needs a referral, you will need to call your new P*P to request that.
If you have an appointment scheduled with me after April 25, 2025, please note that your appointment has been canceled. Beth Israel Lahey Health will have access to your medical records after April 25, 2025, and will have the ability to share the records at your request with any of the Beth Israel Lahey Health Primary Care practices. Should you require a copy of your medical records, please complete the enclosed medical record release form and send it to your new primary care.
