Community Health Access Network The Community Health Access Network (CHAN) is the only Health Center Controlled Network (HCCN) in New Hampshire.

CHAN supports an electronic health record and system infrastructure for its 11 Federally Qualified Health Center (FQHC) members. Our Mission
To enable our member agencies to develop the program and resources necessary to assure access to efficient, effective quality health care for all clients in our communities, particularly the uninsured, Medicaid, and medically underserved populations.
