Not all facelifts are created equal—and the difference is more than just terminology.

Both SMAS and deep plane facelifts address the SMAS layer, the deeper structural layer beneath the skin that plays a major role in facial aging. But what many patients don’t realize is that this layer behaves differently depending on where you are in the face.

Along the outer face, the SMAS is relatively fixed and less mobile. As you move inward toward the cheeks, nasolabial folds, and mouth, it becomes far more mobile—and this is where many of the most visible signs of aging develop.

A traditional SMAS facelift primarily lifts from the more lateral (outer) face, improving contour by tightening and repositioning tissue in areas with less mobility.

A deep plane facelift takes this a step further by releasing deeper facial retaining ligaments and repositioning tissue more medially—closer to where aging actually occurs.

Why does this matter?

Because facial aging is not just about loose skin. It’s about deeper structural descent. Addressing the tissues that have truly shifted often allows for a more natural, balanced rejuvenation without an overly tight or operated appearance.

The technique matters—but more importantly, understanding “what” is being lifted, “where”, and “how” is what ultimately shapes the result.
